The next wave of GLP-1 therapies aims to improve upon existing treatments in terms of both efficacy and convenience. Retatrutide, developed by Eli Lilly, is a weekly injectable that combines GLP-1, GIP, and glucagon – earning it the moniker “Triple G.” Clinical trial results have been remarkable, demonstrating an average weight loss of up to 29% (approximately 71 pounds) after 68 weeks in individuals with knee osteoarthritis. Novo Nordisk is also pursuing “Triple G” therapies, reporting average weight loss of nearly 20% after 24 weeks in a mid-stage study conducted in China. The company is also investigating CagriSema, a combination of semaglutide and cagrilintide, though its results have not yet matched those of Lilly’s tirzepatide or retatrutide.
Convenience and Cost: The Wegovy Pill and Orforglipron
The introduction of oral GLP-1 medications is addressing key barriers to access: convenience and cost. The Wegovy pill, priced at $499 per month for cash-paying patients, offers a more affordable option compared to the list price of approximately $1,350 for the injectable version. Eli Lilly’s forthcoming orforglipron is expected to offer even greater convenience, as it can be taken at any time of day, with or without food and drink, unlike Wegovy, which requires specific timing and restrictions.
The Wegovy pill must be taken first thing in the morning with a slight amount of water, and no food, drinks, or other medications for at least 30 minutes. Orforglipron’s flexibility is expected to improve patient adherence.
Addressing Access and Responsible Use
Despite advancements in treatment and pricing, access to GLP-1 medications remains a challenge for many. Insurance coverage can be inconsistent, leading some patients to seek direct-pay options or switch to the Wegovy pill. Blue Cross Blue Shield of Massachusetts recently announced it would stop covering GLP-1s for obesity due to cost concerns, suggesting patients consider purchasing the drugs directly from manufacturers.
