The Food and Drug Administration (FDA) approved enlicitide, a once-daily oral tablet taken on an empty stomach that lowers low-density lipoprotein (LDL) cholesterol by nearly 60 percent. According to medical data, the medication matches the efficacy of the strongest injectable drugs on the market while eliminating the need for needles or specialist prescriptions.

Cardiovascular disease remains the leading cause of death in the United States and globally. In 2025, the condition claimed 694,708 American lives, accounting for roughly one in five deaths and surpassing all forms of cancer combined, according to federal health statistics. Worldwide, cardiovascular disease kills approximately 20 million people annually. However, age-adjusted death rates in the US have fallen by about three-quarters since 1950, driven by significant improvements in managing LDL cholesterol, smoking cessation rates, and blood pressure control.

The Genetic Blueprint Behind Modern Cholesterol Drugs

The scientific mechanism behind enlicitide originates from research conducted in the early 2000s by geneticists Helen Hobbs and Jonathan Cohen at the University of Texas Southwestern. By analyzing the Dallas Heart Study, the researchers discovered that individuals carrying a broken copy of the PCSK9 gene cleared LDL cholesterol from their blood with unusual efficiency. Carriers of this specific genetic variant exhibited roughly 28 percent lower LDL levels and an 88 percent lower risk of heart disease compared to non-carriers.

That discovery provided drugmakers with a clear biological target. Every PCSK9 drug since, including enlicitide, replicates the protective effects observed in those study participants. While traditional statins remain widely prescribed, some patients experience muscle aches or fail to reach target LDL reductions. Enlicitide offers an alternative option for patients who require high-potency cholesterol reduction without injections or statin-related side effects.

Future Pipeline: Gene Editing and Patient Adherence

Despite the potency of daily medications, patient adherence remains a significant hurdle in cardiology. According to clinical data, approximately half of all patients prescribed statins discontinue use within the first year. To address adherence challenges, developers are exploring single-infusion genetic treatments.

Verve Therapeutics, a company owned by Eli Lilly, has evaluated a treatment that introduces a single-letter edit to the PCSK9 gene in the liver using a single infusion. Results from its first human trial, published in the New England Journal of Medicine, showed that one dose reduced LDL cholesterol by up to 62 percent and sustained that reduction for over a year by switching off the target gene.

Smoking rates among US adults declined from roughly 40 percent in the 1960s to under 15 percent, while anti-obesity GLP-1s have demonstrated cardiovascular benefits, with clinical trials showing semaglutide reducing cardiovascular events by 20 percent.
