Recent research reveals that heavy drinkers harbor approximately 10 more bacterial species in their mouths compared to abstainers, according to data presented at the Nutrition 2026 congress and a published January 2024 study in the Journal of Nutrition. Led by Samantha Maley at the University of Buffalo, the published research examined subgingival microbiome data from 1,179 postmenopausal women participating in the OsteoPerio study, an ancillary project of the Women’s Health Initiative.

Subgingival Microbiome Diversity and Alcohol Consumption

According to the University of Buffalo study published in the Journal of Nutrition, researchers utilized 16S ribosomal RNA sequencing on subgingival plaque samples collected from 12 teeth per participant. Dietary questionnaires measured alcohol intake, showing that 67 percent of the 1,179 participants consumed alcohol, with 14 percent reporting at least one drink per day. Wine accounted for 66 percent of total alcohol consumed within the sample group. The analysis found that overall subgingival flora composition, known as beta-diversity, varied significantly across drinking levels after adjusting for variables such as age, smoking, body mass index, and dental hygiene habits. Intra-oral species richness was higher among heavy consumers, showing an average of 127 observed species in the group consuming seven or more drinks per week compared to 116 species in abstainers.

Why Increased Bacterial Diversity Signals Risk

Unlike gut microbiomes where high microbial diversity generally indicates good health, subgingival bacterial diversity correlates with the severity of periodontal disease, according to the study’s authors. Researchers suggest that alcohol intake may promote periodontitis by increasing this diversity through specific biological mechanisms. Alcohol weakens the phagocytosis capacity of neutrophils, which are primary antibacterial defense cells, and reduces salivary flow rates. This reduction in saliva acidifies the oral cavity, creating an environment that encourages the proliferation of gum disease-associated bacteria. Massachusetts-based dentist Ambereen Fatima notes that evening alcohol consumption dries the mouth precisely when natural salivary rinsing action ceases, while alcoholic beverages often contain high levels of acid and sugar.

Comparing Beverage Types and Study Limitations

The 2024 Buffalo study evaluated beer, wine, and liquor separately, finding no statistically significant differences in beta-diversity by beverage type. While wine consumption showed an association with intra-oral diversity, participants who drank exclusively wine showed no statistically significant associations across tested indices, which researchers attribute to statistical power since wine comprised two-thirds of total alcohol consumed. The unpublished Nutrition 2026 conference presentation utilized mouthwash rinse samples rather than subgingival plaque and indicated that alcohol remains the sole beverage category linked to broad oral flora shifts, whereas tea, coffee, and sugary drinks showed minimal effect. Study authors acknowledged important limitations, including the cross-sectional design preventing causal conclusions, reliance on self-reported questionnaires, and an observation that removing non-drinkers from the analysis eliminated significant associations between consumption levels and bacterial diversity.
