Researchers investigated how chronic citrus intake impacts peripheral blood mononuclear cells in healthy adults, revealing distinct genetic responses dependent on individual body weight.

The single-arm pre-post intervention trial examined 20 healthy adults aged 21 to 36, consisting of 10 men and 10 women, without chronic disease. Participants underwent a three-day citrus-free washout period before consuming 500 milliliters, or about two cups, of pasteurized orange juice daily for 60 days, split into two home doses. Fasting blood samples were collected at baseline and at day 60 to isolate peripheral blood mononuclear cells for global transcriptomic profiling on Clariom D microarrays.

Transcriptomic Remodeling and Gene Activity

Analysis of the blood samples showed that chronic orange juice intake remodeled the transcriptome of circulating immune cells, altering 3,790 oligonucleotides. This included changes to 1,705 protein-coding genes, with the majority experiencing downregulation, alongside shifts in 66 microRNAs, 19 long non-coding RNAs, and 67 small nucleolar RNAs.

Researchers identified that several affected genes regulate inflammation and lipid handling. Specifically, genes such as IL6 and IL1β, which drive inflammatory pathways, showed reduced impact through downregulation. Other genes involved in processing and utilizing fats, including GSK3B and RIPK1, also shifted in activity levels following the 60-day intervention.

Body Mass Index Differences in Molecular Response

The study found that molecular responses to the daily beverage varied significantly depending on participant body mass index. Normal-weight participants exhibited more pronounced changes in genes connected to inflammatory pathways. In contrast, overweight participants showed primary shifts related to lipid metabolism and energy use.

Scientists attribute these molecular changes to citrus flavanones present in orange juice, such as hesperidin and naringenin. These plant-based antioxidants influence vascular tone, lipid handling, and immune signaling by interacting with transcription factors and altering gene expression in immune cells.

Study Limitations and Context

The investigation was structured as an exploratory, single-arm trial without a control beverage, meaning findings demonstrate transcriptomic associations rather than establishing direct causality. While the mechanistic paper did not newly assess clinical endpoints, prior publications from the same research cohort reported reductions in blood pressure and body-fat percentage following the same 500 milliliter per day intake over 60 days. Authors note that larger, controlled trials are necessary to determine whether these molecular alterations translate into measurable, long-term health improvements.
