A recent clinical trial published in Cell Metabolism reveals that a ketogenic diet reduces liver fat and improves metabolic markers in adults with obesity more significantly than a Mediterranean diet or a low-fat, plant-based diet, despite all three diets resulting in comparable weight loss.

Clinical Trial Design and Macronutrient Breakdown

To ensure strict adherence, the research team provided all meals directly to the participants, who also met weekly with a registered dietitian to monitor their macronutrient distribution over a four-to-five-month period. The primary objective for each group was to achieve approximately a 10 percent reduction in total body weight.

The dietary intervention divided the cohorts into distinct macronutrient profiles. According to the study data, the ketogenic group derived 73 percent of their daily calories from fat and only 4 percent from carbohydrates. By comparison, the Mediterranean diet group consumed 50 percent of calories from carbohydrates and 35 percent from fat, while the low-fat, plant-based group ate 70 percent carbohydrates and 15 percent fat. Continuous 24-hour metabolic monitoring via repeated blood draws showed that the ketogenic diet triggered the largest increase in glucagon, a hormone that stimulates the release of energy stores from the liver.

Metabolic Outcomes and Prediabetes Reversal

Despite consuming a high-fat diet, participants in the ketogenic arm did not experience adverse spikes in blood lipids. According to Max Petersen, the most surprising finding was that participants in the ketogenic diet group didn’t experience an increase in blood fat or cholesterol levels despite consuming the most fat. Concurrently, the data indicated a sharper decline in circulating insulin levels among the keto cohort, which researchers associate with a more rapid clearance of hepatic fat.

These physiological shifts translated into tangible clinical improvements. At the conclusion of the study, approximately half of the participants in the ketogenic group no longer met the clinical criteria for prediabetes. In contrast, prediabetes remission occurred in 29 percent of the Mediterranean diet group and 7 percent of the low-fat, plant-based control group.

Expert Perspectives and Study Limitations

Independent endocrinologists have emphasized both the rigorous control and the inherent limitations of the research. Yesika Garcia, an endocrinologist not involved in the trial, described the investigation as a small but well-designed and controlled study that demonstrates how dietary composition can independently influence liver health even when overall weight loss is equal. Garcia noted that the trial’s small cohort of 42 individuals and its five-month duration mean the findings cannot be immediately generalized to the broader population.

Furthermore, the study observed no significant differences in muscle mass retention across the three dietary interventions. Because skeletal muscle functions as the body’s primary glucose-disposal tissue, the preservation of lean mass remained uniform regardless of whether participants followed a high-fat keto regimen, a Mediterranean diet, or a high-carbohydrate plant-based plan.

The trial underscores that while total caloric reduction drives weight loss, specific macronutrient structures exert distinct biochemical effects on hepatic and metabolic health. As researchers continue to evaluate how targeted nutritional therapy impacts conditions like fatty liver disease and prediabetes, future studies will need to assess long-term adherence and larger patient populations to confirm these metabolic advantages over extended periods.
