Heavy marijuana smoking may increase the risk of developing both small cell and non-small cell lung cancer, along with several types of head and neck cancers, according to recent medical research. As cannabis legalization expands across the United States, thoracic surgeons and otolaryngologists are examining how daily use and combustion impact long-term oncological health, drawing distinct lines between smoked products and non-combustible alternatives like edibles.

Evaluating the Link Between Heavy Marijuana Use and Lung Cancer

While the health consequences of tobacco smoking are well-documented, the clinical relationship between cannabis use and pulmonary malignancies remains a subject of ongoing investigation. According to Brooks Udelsman, MD, a thoracic surgeon with USC Surgery at Keck Medicine of USC, determining exact risk profiles depends heavily on consumption patterns and dosage. A recent Keck Medicine study led by Niels Kokot, MD, an otolaryngologist with the USC Caruso Department of Otolaryngology – Head and Neck Surgery, indicates that individuals who smoke marijuana heavily face an elevated risk of developing both small cell and non-small cell lung cancer.

Researchers are still working to quantify the precise dose relationship. Dr. Udelsman notes that while occasional users—those who consume cannabis once a week, once a month, or a few times a year—likely carry minimal risk, patients who smoke heavily to the point of dependency or require hospital evaluation show a notably higher cancer risk. In addition to pulmonary risks, a separate study led by Dr. Kokot reveals that daily marijuana users are 3.5 to 5 times more likely to develop head and neck cancers compared to nonusers. These malignancies encompass the mouth, pharynx, larynx, oropharynx including the tongue, tonsils, and back wall of the throat, as well as nearby salivary glands.

Chemical Mechanisms and the Impact of Smoke

The carcinogenic potential of smoked marijuana stems from the physical process of combustion and the resulting chemical exposure. Dr. Udelsman explains that any agent causing long-term cellular inflammation can raise cancer risk. While tobacco smoke contains more than 7,000 chemicals with approximately 70 linked to cancer, marijuana smoke shares several of these harmful compounds.

Furthermore, tetrahydrocannabinol (THC), the primary psychoactive component in marijuana, connects to the conversion of polycyclic aromatic hydrocarbons (PAHs). These PAHs can trigger chronic inflammation and cause direct DNA damage. Dr. Udelsman emphasizes that whenever inflammation and DNA alterations occur simultaneously, the biological risk for cancer development increases.

Comparing Consumption Methods and Secondary Exposure

A critical distinction in current research lies in how cannabis is consumed. When asked whether non-smoking methods such as edibles carry similar pulmonary risks, Dr. Udelsman states that they probably do not. While tracking data for edibles remains scarce due to a lack of long-term studies, current clinical observations show no established relationship between ingestion methods and lung cancer, though links to other cancer types remain unstudied.

Regarding secondhand smoke, researchers report that definitive evidence is currently lacking to confirm a direct link between secondhand marijuana smoke and lung cancer. However, Dr. Udelsman cautions that breathing in secondhand smoke still exposes individuals to inflammatory particulate matter associated with oncogenesis. Researchers are also investigating whether heavy cannabis use correlates with other malignancies, such as bladder and gastrointestinal cancers, mirroring patterns observed in long-term tobacco studies.
