The Origins and Criteria of the Apgar Score

In 1952, Virginia Apgar invented the rapid and reliable assessment tool known as the Apgar score, filling a major gap in post-delivery medicine where newborn distress often went unnoticed until conditions turned critical. An attending nurse, midwife, or physician evaluates five specific criteria: skin color, heart rate, reflex irritability, muscle tone, and respiration. The test is administered twice—at one minute and five minutes after birth, with additional assessments if the infant’s condition requires close monitoring. Each criterion receives a score of zero, one, or two points.

Scores ranging from 7 to 10 are considered reassuring, while 4 to 6 indicate an intermediate status and 0 to 3 are classified as low. Many newborns register lower scores at the one-minute mark but recover normal strength and coloring by the five-minute evaluation. Despite the widespread adoption of the test, Apgar’s direct contribution to modern medicine remained underrecognized by the general public, largely because her surname was later converted into an acronym matching her five observation points: Appearance, Pulse, Grimace, Activity, and Respiration.

Advancing Anesthesiology and Congenital Research

Beyond her neonatal scoring system, Apgar played a central role in establishing anesthesiology as a formal medical specialty, improving obstetric anesthesia methods, and shifting the medical understanding of birth defects. She served as the director of the anesthesiology division at Columbia University, marking the first time a woman held a full professorship there. Her dedication to emergency response was well known; she routinely carried a resuscitation kit containing a scalpel and an endotracheal tube, famously remarking that no one should ever stop breathing in her presence.

In the late 1950s, Apgar shifted her primary focus toward the study and prevention of congenital malformations. She joined the National Foundation for Infantile Paralysis—later renamed March of Dimes—serving as director and subsequently as vice-president. In this role, she investigated how infectious diseases, radiation exposure, and chemical substances influence birth anomalies, while traveling across the United States to raise public awareness.

Musical Luthier and Multitalented Pioneer

Outside of her medical career, Apgar pursued diverse interests, including gardening, fly-fishing, and aviation training. She was also an accomplished violinist who played in multiple chamber orchestras and learned to build stringed instruments, including violins, a viola, and a cello. Her work as a luthier once led her to participate in the covert removal of a maple shelf from a telephone booth at the Columbia University Medical Center in 1957 to secure raw material for a viola. That instrument, along with three others she crafted, was played during a 1994 ceremony commemorating the release of a commemorative stamp issued in her honor. Virginia Apgar died in 1974 at the age of 65 due to hepatic failure, leaving behind a legacy that continues to safeguard newborn health globally.
