"Aging used to just be aging," Hoenig told Bild. "Today it means for me that I am still allowed to live. And I am grateful for that. That makes me humble." He added that there were times when he doubted he would survive until the next day, stating he is filled with happiness to sit alongside his wife and sons.

Battling Ongoing Complications After Surgery

Following his initial hospitalization for acute heart issues in 2024 and subsequent time in a medically induced coma, Hoenig underwent surgery on his esophagus. In mid-September, he experienced renewed heart complications requiring additional hospital treatment.

According to Bild, the actor has been fed via a feeding tube since his esophageal operation. Addressing his recovery, Hoenig shared that he misses eating normal food independently. "I don’t need gourmet food," he told Bild. "A simple buttered bread would make me insanely happy."

Family Devotion and Instagram Tribute

Hoenig shares two young sons, five-year-old Juliano and three-year-old Jianni, with his 41-year-old wife, Annika Kärsten-Hoenig. Reflecting on his near-death experience, Hoenig expressed a fear of leaving his family behind prematurely. "My boys are still small, I want to be their dad for a long time. And I don’t want to leave my Anni alone either," he said, noting that watching his sons grow and spending many more years with his wife represents his ultimate wish.

To mark his milestone birthday, Annika Kärsten-Hoenig posted a tribute to her husband on Instagram. "With you I was allowed to experience what a home feels like when it doesn’t mean a place," she wrote. "From us became a family, from love became life—and from all the moments something I never want to miss again. Happy Birthday to my man, my best friend, the papa of our sons, and my very great love."

Hoenig rose to international prominence for his role in the acclaimed 1981 war film Das Boot. Throughout the 1990s, he became a fixture of German television through major multi-part broadcasts directed by Dieter Wedel, including Der große Bellheim, Der Schattenmann, and Der König von St. Pauli.
