Methylphenidate and Psychosis Risk in ADHD: New Research Offers Reassurance
A recent study published in JAMA Psychiatry suggests that methylphenidate, a common stimulant used to treat attention-deficit/hyperactivity disorder (ADHD), may not increase the risk of psychosis in children and adolescents. In fact, sustained treatment with methylphenidate in children under 13 appeared to lower the risk of developing psychotic conditions.
ADHD and Psychosis: Understanding the Connection
Children with ADHD have historically been identified as having a potentially higher risk of developing psychotic disorders like bipolar disorder or schizophrenia compared to neurotypical children. Some previous research had suggested a possible link between stimulant medications for ADHD and an increased risk of psychosis, leading to concerns among parents and clinicians.
The New Study: A Large-Scale Analysis
Researchers from the University of Edinburgh and University College Dublin analyzed the health records of nearly 4,000 children and adolescents in Finland diagnosed with ADHD. The findings indicated no difference in psychosis risk between those treated with methylphenidate – the most commonly prescribed medication for ADHD – and those who were not. Approximately 3.5 million children between the ages of 3 and 17 in the United States are currently prescribed ADHD medications. [1]
Why Methylphenidate May Be Protective
Dr. Ian Kelleher, chair of child and adolescent psychiatry at the University of Edinburgh and the study’s senior author, explained that the increased risk of psychosis in individuals with ADHD may be due to shared genetic risk factors between the disorders, independent of medication. “We found overall that whereas we know that this group is at an increased risk of psychosis, it wasn’t to do with the medication,” Kelleher stated. The Role of Dopamine and Potential Risks in Later Adolescence
Dr. Christian Kohler, professor of psychiatry at the Hospital of the University of Pennsylvania, described the results as “thought-provoking and not expected.” However, he cautioned that the findings may not apply to older age groups. Stimulants increase the availability of dopamine in the limbic system, a brain region involved in emotional regulation, which could potentially contribute to erratic thinking and hallucinations in vulnerable individuals.
Kohler suspects a potential “tipping point” may exist, possibly starting in the late teens, where the risk of psychosis could increase following stimulant treatment. He noted instances of individuals newly diagnosed with ADHD in their late teens experiencing psychosis within months of starting stimulant medication.
Amphetamines vs. Methylphenidate: A Crucial Distinction
The study focused specifically on methylphenidate (Ritalin, Concerta). Researchers acknowledged the need for similar analysis with amphetamines (Adderall, Vyvanse), a different class of stimulants more commonly prescribed to adults and privately insured patients. Research suggests patients prescribed amphetamines are more likely to develop psychosis than those prescribed methylphenidate. [3]
Dr. Jeffrey Newcorn, director of the division of ADHD and learning disorders at the Icahn School of Medicine at Mount Sinai, emphasized that psychosis risk is generally considered higher in patients prescribed amphetamines. A 2024 study found that high doses of amphetamines were associated with a more than fivefold greater risk of developing psychosis or mania. [4]
Factors Influencing Psychosis Risk
Kohler believes that medication dose, timing of treatment, and the patient’s overall stability likely play a critical role in determining whether stimulants are protective or contribute to psychosis risk. He highlighted previous research in animals showing that long-term methylphenidate treatment in juvenile animals can normalize abnormalities in dopamine-rich brain regions, suggesting a potential preventative effect.
Looking Ahead
While further research is needed, the study offers reassurance to parents of children with ADHD. “It’s reassuring that the risk of a serious lifelong psychotic disorder is not increased by [this] medication,” Newcorn said. [5]
