Unlike standard interventions that only temporarily reverse acute crises, monthly agonist treatments offer sustained protection for patients navigating addiction and recovery.

The Hidden Cognitive Toll of Non-Fatal Overdoses

Every minute that passes between an opioid overdose and a resuscitating injection brings a patient closer to brain injury. Dr. Nathaniel Day noted that while individuals often survive individual overdoses, repeated episodes of oxygen deprivation progressively damage the brain. According to data cited by Day from British Columbia, between 6 and 10 per cent of individuals who experience a first overdose show up in the health-care system within a year with a formally diagnosed brain injury. These cumulative micro-injuries accumulate over time, leaving patients with cognitive impairments that directly undermine their ability to navigate treatment and recovery. Day explained that because these injuries stack up gradually, many patients suffer from chronic, permanent damage without a single, immediately diagnosable trauma event.

Sublocade as a Monthly Shield Against Brain Injury

To combat the cycle of repeated overdoses and stacking brain injuries, medical experts are advocating for the expanded use of Sublocade, a once-a-month injectable buprenorphine formulation. According to Dr. Day, long-acting injectables provide a steady baseline of medication that helps keep a person breathing even if they encounter fentanyl. Day co-authored a letter published in the journal Addiction alongside other Canadian experts, including Dr. Daniel Vigo, the B.C. premier’s chief scientific adviser for psychiatry, toxic drugs & concurrent disorders. The expert letter emphasizes that standard addiction care has historically focused almost exclusively on preventing death, often overlooking the urgent need to protect the brain from chronic injury or adapt treatment protocols for patients already living with cognitive deficits.

Dr. Day points to structured programs like Alberta’s Virtual Opioid Dependency Program—which offers free, same-day access to care—as vital pathways for connecting vulnerable individuals with long-acting treatments. In July, Recovery Alberta announced a $30-million plan encompassing compassionate (involuntary) intervention and involuntary retention measures for individuals and youths dealing with damaging addictive issues.
