Maintaining normal blood pressure, avoiding diabetes, and refraining from smoking between the ages of 48 and 68 are associated with nearly 13 additional years of dementia-free life, according to a study published on August 5 in Neurology Open Access. Led by researchers at NYU Langone Health, the findings highlight how targeted vascular risk reduction during middle age can significantly delay the onset of cognitive decline.
The Link Between Vascular Health and Dementia-Free Survival
According to data from the community-based Atherosclerosis Risk in Communities (ARIC) Study, participants who maintained zero vascular risk factors at age 55 could expect to live an average of 30 years without developing dementia. By contrast, individuals burdened with all three risk factors—high blood pressure, diabetes, and smoking—experienced drastically shortened cognitive health spans, averaging just over 17 years of dementia-free life from the study’s start.

The study analyzed 12,409 participants with an average age of 56 who were free of dementia at baseline. Over an average follow-up period of 26 years, researchers documented 3,008 cases of incident dementia, while 5,238 participants died without developing the condition. Participants with all three midlife risk factors faced nearly three times the hazard of developing dementia compared to those with clean vascular profiles.
“Our findings argue that people need to actively avert these factors in midlife as a strategy for preserving brain health for more than a decade,” said study senior investigator Josef Coresh, founding director of the Optimal Aging Institute and professor at NYU Langone.
Demographic Differences in Dementia-Free Lifespans
The research team also evaluated how midlife cardiovascular risk factors influenced dementia-free survival across different demographic groups. Across all categories, women generally lived longer without dementia than men. Among participants with all three risk factors, women averaged 18.1 dementia-free years, compared to 16.6 years for male study participants.

Disparities also emerged along racial lines within the cohort. White participants with all three risk factors lived an average of 19.6 years without developing dementia, compared to 16.0 years for Black participants. Coresh noted that while vascular risk reduction benefits everyone, certain populations, such as Black adults, may derive substantial advantages from targeted prevention efforts.
Public Health Implications and Study Limitations
With the World Health Organization projecting that global dementia cases will surpass 150 million by 2050, identifying modifiable midlife risk factors remains a central focus for researchers. Coresh emphasized that the results should encourage people to quit smoking and monitor their vascular health closely starting at age 48.
Study authors noted certain limitations in their design. The observational study was not structured to prove direct causation between avoiding vascular risk factors and delayed dementia onset. Additionally, risk factors were measured at a single point in time, meaning shifts or interventions later in life were not captured.
