Routine Brain MRIs May Detect Breast Cancer Metastases Earlier

Regular brain magnetic resonance imaging (MRI) screening may detect asymptomatic brain metastases in patients with advanced HER2-positive or triple-negative breast cancer, potentially improving treatment outcomes and quality of life, according to a recent study.

Early Detection of Brain Metastases

Researchers at Yonsei Cancer Center in South Korea analyzed 112 patients with advanced HER2-positive or triple-negative breast cancer who did not exhibit neurological symptoms of brain metastasis between 2018 and 2023. MRI scans were conducted at the time of diagnosis and during subsequent lines of therapy.

Initial scans revealed brain metastases in 9.8% of patients despite the absence of symptoms. Continued screening increased the cumulative detection rate to 19.6%. Approximately 67% of patients who ultimately developed brain metastases were diagnosed before experiencing neurological symptoms. Source

Treatment and Quality of Life

Patients diagnosed with brain metastases were treated with stereotactic radiosurgery (SRS), a non-invasive radiation therapy. Cognitive assessments showed no significant decline in function before or after treatment, suggesting early intervention may help preserve quality of life.

Current Guidelines and Future Implications

Current clinical practice generally does not recommend routine brain MRI screening for advanced breast cancer patients unless they present with neurological symptoms. The study’s findings challenge this approach, particularly for high-risk subtypes like HER2-positive and triple-negative breast cancer, which are known to have a higher propensity for brain metastasis.
