China is facing a sharp rise in lung cancer deaths, with fatalities projected to reach 760,200 by the year 2030, according to a comprehensive mortality analysis published in Cancer Biology & Medicine. Researchers from the National Cancer Center (China), the Chinese Academy of Medical Sciences, and Peking Union Medical College attribute this mounting death toll primarily to a rapidly aging population rather than worsening risk factors.
Demographic Shifts Mask Progress in Mortality Rates
Between 2013 and 2021, the crude lung cancer mortality rate in China climbed by 2.3% annually, according to data from the China Causes of Death Surveillance Dataset, which evaluated 2.37 billion person-years across 605 surveillance sites. However, the study’s authors—led by Xin Liang and Xiaoqiu Dai—uncovered a critical paradox. When adjusting for the aging population, the age-standardized mortality rate remained stable overall and decreased by 2.9% per year after 2015.
Decomposition analysis within the study revealed that population aging drove between 32% and 43% of the increase in total lung cancer deaths. Population growth contributed an additional 8% to 31%. Conversely, changes in underlying risk factors actually reduced deaths by 3% to 29%, signaling that national interventions are yielding results despite the growing raw numbers.
Regional Disparities in Cancer Outcomes
The research team identified distinct geographic variations in how lung cancer mortality is evolving across the country. Urban areas experienced the sharpest declines in age-standardized mortality rates, dropping by 2.9% annually, while eastern regions saw a 1.5% decrease.
At the same time, the mean age at death from lung cancer increased across all regions. Rural areas recorded the fastest rise in the age of death, with men gaining roughly 0.27 years annually and women gaining 0.30 years. The authors noted that the primary challenge moving forward is ensuring that prevention and treatment systems keep pace with demographic shifts, particularly in rural and western areas where health gains have arrived more slowly.
Risk Factors and Future Projections
Lung cancer remains the leading cause of cancer-related death globally and in China, accounting for approximately 40% of the world’s lung cancer deaths in 2022 despite the country housing only about 18% of the global population. Persistent risk factors compound the challenge; historical data shows that 26.6% of Chinese adults smoked in 2018, and 68.1% of non-smokers faced secondhand smoke exposure in public spaces.

With the proportion of Chinese citizens aged 60 and older projected to surpass 500 million by 2050, researchers emphasize that tobacco control efforts must intensify in rural and western regions where smoking rates remain high. Furthermore, expanding low-dose spiral computed tomography screening into grassroots and rural health facilities—potentially utilizing mobile units and artificial intelligence interpretation—remains a central recommendation to catch the disease earlier among vulnerable older populations.
