Understanding Diabetes Mellitus: Types, Symptoms, and Complications
Diabetes mellitus, a chronic metabolic disease characterized by elevated blood glucose levels, is a growing global health concern. In Indonesia, it ranks as the third leading cause of death [1]. Understanding the different types, recognizing the symptoms, and being aware of potential complications are crucial for effective management and prevention.
Classifications of Diabetes Mellitus
According to Dr. Wirawan Hambali, Sp. P.D, FINASIM, there are four main classifications of diabetes mellitus [1], [2]:
- Type 1 Diabetes: This occurs due to the destruction of insulin-producing beta cells in the pancreas, leading to absolute insulin deficiency. The cause can be autoimmune or idiopathic (unknown). It often develops in younger individuals and is associated with a lean body type.
- Type 2 Diabetes: This is the most common form, accounting for approximately 95% of cases [1]. It’s characterized by insulin resistance, where the body’s cells don’t respond effectively to insulin, and often linked to obesity.
- Gestational Diabetes: This develops during pregnancy.
- Specific Types of Diabetes: This category encompasses diabetes caused by other specific factors.
Recognizing the Symptoms
The classic symptoms of diabetes are often described by the “4 Ps”:
- Polyuria (Frequent Urination): High blood sugar levels cause the kidneys to excrete excess glucose through urine, leading to increased urination. The urine may even attract ants due to its high sugar content.
- Polydipsia (Excessive Thirst): Increased urination leads to dehydration, triggering intense thirst.
- Polyphagia (Increased Hunger): Since sugar cannot enter the cells effectively, the body experiences energy deprivation, leading to increased appetite.
- Weight Loss: Despite increased food intake, individuals with untreated diabetes may experience unintentional weight loss as the body cannot utilize glucose for energy.
Short-Term and Long-Term Complications
Diabetes can lead to both short-term and long-term complications:
- Short-Term Complications: These typically arise from drastic fluctuations in blood sugar levels and can include diabetic coma, a medical emergency.
- Long-Term Complications: Uncontrolled high blood sugar can damage blood vessels and nerves over time, leading to:
- Microvascular Complications: Damage to modest blood vessels, resulting in blindness and kidney problems.
- Macrovascular Complications: Damage to large blood vessels, increasing the risk of coronary heart disease and stroke.
- Non-Vascular Complications: Including fatty liver and intestinal motility disorders.
Risk Factors and Prevalence in Indonesia
Several factors contribute to the risk of developing diabetes mellitus, including obesity, central obesity, high consumption of sugary foods and beverages, fatty foods, lack of physical activity, and smoking [3]. A study utilizing data from the Indonesian Family Life Survey-5 (IFLS-5) in 2014 found that 8.4% of respondents had diabetes [4].
Efforts to control these risk factors, particularly obesity and central obesity, are essential for preventing and managing diabetes in Indonesia.
