England Plans Strict Licensing Rules for High-Risk Cosmetic Procedures by 2027

England will ban under-18s from receiving high-risk cosmetic treatments and restrict procedures such as liquid Brazilian butt lifts (BBLs) to doctors under government proposals scheduled for spring 2027. The regulatory overhaul targets unregulated operators offering “no scalpel” procedures in settings like private homes and hotel rooms, bbc.co.uk reported.

Under the draft rules, dermal fillers injected into areas other than the face—such as liquid BBLs—will be legally classified as cosmetic surgical procedures. This classification restricts the injections exclusively to doctors. High-risk non-surgical or wellbeing treatments, including deep chemical peels, vampire facials, dermal micro-coring, and some laser procedures, will be limited to doctors, dentists, and some trained nurses, midwives, and pharmacists.

The proposed licensing scheme responds to severe medical complications and fatalities linked to rogue operators. In 2024, Alice Webb, a mother of five from Gloucestershire, died within hours of undergoing a liquid BBL procedure. That same year, Sasha Dean from Bedfordshire suffered a heart attack, lung collapse, and kidney failure after developing sepsis from a liquid BBL administered in a private house.

Health Minister Karin Smyth stated that the government intends to drive “cosmetic cowboys” out of the market and establish higher standards to help consumers identify safe, qualified providers. The British Beauty Council praised the initiative for confronting the issue directly. However, Keith Allison from the Royal College of Surgeons of England argued that the proposals do not go far enough, warning that officials must avoid creating new regulatory loopholes between non-surgical procedures and traditional cosmetic surgery.

What Are the Rules for Scotland and Wales?

Regulatory frameworks across the United Kingdom vary significantly by region. In Scotland, legislation enacted in March mandates that cosmetic procedures must be performed by or alongside certain healthcare professionals within a registered setting, while simultaneously banning non-surgical treatments for individuals under 18. Conversely, Wales and Northern Ireland maintain no dedicated regulations governing cosmetic procedures.
