Adults with type 2 diabetes who take GLP-1 receptor agonist medications experience a significantly lower one-year mortality rate, with patients also diagnosed with obstructive sleep apnea seeing a disproportionate survival benefit, according to a study presented by researchers at the American College of Chest Physicians meeting in Chicago.

The findings, reported in October 2025, offer fresh clinical context following the U.S. Food and Drug Administration’s December 2024 decision to approve Zepbound (tirzepatide) as the first prescription drug specifically indicated for obstructive sleep apnea in adults with obesity. Dr. Cosmo Fowler, a sleep medicine physician at the Piedmont health system in Atlanta and lead researcher on the study, noted that obstructive sleep apnea appears to act as an effect modifier in the association between GLP-1 prescriptions and survival outcomes.

Mortality Benefits in Diabetes and Sleep Apnea Patients

Researchers analyzed health data covering nearly 1.8 million patients diagnosed with type 2 diabetes, of whom roughly 28% had active prescriptions for GLP-1 receptor agonists, according to findings detailed by HealthDay. While the data showed that all diabetes patients taking GLP-1 medications experienced a lower short-term risk of death compared to those not taking the drugs, the protective effect was more pronounced among individuals with concurrent sleep apnea.

Patients with sleep apnea who took GLP-1 medications faced a 20% lower risk of death over a one-year period compared to diabetes patients with sleep apnea who were not prescribed the drugs. Among patients without sleep apnea, the absolute risk of death stood at 0.9% for those on GLP-1 therapy versus 1.8% for untreated patients. By comparison, among patients with sleep apnea, the one-year mortality risk was 1% for those taking GLP-1 medications compared to 2.5% for those without them.

Mechanisms Linking GLP-1 Drugs to Sleep Apnea Relief

Obstructive sleep apnea occurs when a person’s upper airway becomes blocked while they snooze, causing pauses in their breathing that awaken them. Obesity remains the single largest modifiable risk factor for the condition, as excess weight from fatty deposits increases pressure on the upper airways, causing them to collapse, and reduces a person’s lung capacity.

GLP-1 medications such as Ozempic, Wegovy, and Zepbound achieve average weight losses of 12% to 22% over a one-to-two-year period. Clinical trials such as SURMOUNT-OSA demonstrated that patients using tirzepatide reduced their apnea-hypopnea index by an average of 25 events per hour at the one-year mark. While researchers suggest minor direct effects on upper airway muscle tone and inflammation may contribute, the primary driver of symptom reduction is weight loss.

Clinical Guidelines and Testing Requirements

Current FDA labeling for Zepbound restricts the sleep apnea indication to adults with a body mass index of 30 or higher and a confirmed diagnosis of moderate-to-severe obstructive sleep apnea, defined as an apnea-hypopnea index of 15 or higher measured via polysomnography or a home sleep test. A formal sleep study remains a mandatory diagnostic prerequisite for patients seeking treatment targeted specifically at sleep apnea.

Physicians evaluating patients for GLP-1 therapy are encouraged to consider whether a person has sleep apnea when deciding whether to prescribe a GLP-1 drug. However, clinical pathways do not treat medications as an immediate replacement for continuous positive airway pressure devices. CPAP therapy provides immediate airway protection on the first night of use, whereas improvements from GLP-1 medications lag behind gradual weight loss, prompting many to recommend combination therapy followed by repeat diagnostic testing after six to 12 months.
