GLP-1 Drugs Show Promise in Combating Substance Leverage Disorders
Blockbuster GLP-1 drugs, initially developed for the treatment of diabetes and obesity, are now showing potential in preventing and reducing the harm associated with multiple substance use disorders, according to a recent study published in a medical journal. The findings suggest these medications may target the underlying causes of cravings, offering a new approach to addiction treatment.
How the Study Was Conducted
Researchers analyzed electronic health records from over 600,000 U.S. Department of Veterans Affairs patients with diabetes over a three-year period. The study, one of the largest to date, compared individuals treated with GLP-1 drugs to those receiving other medications for blood sugar control. Patients were divided into seven trials examining the risk of developing addictions to substances including alcohol, cannabis, cocaine, nicotine, and opioids. An additional trial assessed the risk of adverse outcomes—such as emergency department visits, hospitalizations, overdose, and suicidal thoughts—among those already struggling with addiction.
Key Findings
The analysis revealed that individuals initiating GLP-1 drug treatment had a lower risk of developing addiction to several substances compared to those on other medications:
- 18% reduced risk of alcohol addiction
- 14% reduced risk of cannabis addiction
- 20% reduced risk of cocaine and nicotine addiction
- 25% reduced risk of opioid addiction
For patients already with substance use disorders, starting GLP-1s was associated with significant reductions in negative outcomes:
- 31% lower risk of emergency department visits
- 26% lower risk of hospitalizations
- 25% lower risk of suicidal thoughts or attempts
- 39% lower risk of overdose
- 50% lower risk of death
the study estimated that GLP-1 drug use likely prevented approximately seven cases of substance use disorder and 12 incidents of serious harm for every 1,000 users over three years .
How GLP-1s May Work
GLP-1s (glucagon-like peptide-1 receptor agonists) mimic a naturally occurring hormone in the small intestine. These drugs work by influencing appetite, insulin release, digestion, and blood sugar levels . In the context of addiction, researchers believe GLP-1s may reduce cravings by targeting the brain’s reward pathways, similar to how they reduce “food noise” – intrusive thoughts about food – in diabetes and weight management trials. Dr. Lorenzo Leggio, a clinical director at the National Institute on Drug Abuse, suggested the drugs may also reduce “alcohol or drug noise.”
Study Limitations
The study was conducted within the VA health system, primarily serving an older, white, and male population, though the results remained consistent across over 35,000 women. Data was limited to individuals with diabetes, and the analysis couldn’t account for factors like socioeconomic status or lifestyle choices. The study demonstrated an association between GLP-1 use and reduced risk, but did not prove causation. It compared GLP-1s to another drug, not to no treatment.
Future Research
While the findings are promising, researchers emphasize that GLP-1 drugs should not yet be prescribed for the prevention or treatment of substance use disorders outside of clinical trials. Randomized, controlled clinical trials comparing GLP-1s to placebo are needed to confirm these results. Several such trials are currently underway . The ultimate goal is to develop new and effective treatments for addiction, a leading cause of illness and death worldwide.
