People taking popular GLP-1 obesity medications who stay on low starter doses achieve measurable weight loss while experiencing significantly less nausea than patients who graduate to higher manufacturer-recommended doses, according to a study reported on Monday in Biology Methods & Protocols. Researchers analyzed real-world patient data to evaluate the efficacy and adverse event profiles of sustained low-dose regimens for injectable semaglutide and tirzepatide.

Study Findings on Low-Dose GLP-1 Weight Loss

According to the study analyzed by data analytics firm nference, patients taking injectable semaglutide at the 0.25-milligram starter dose for at least six months lost an average of 2.2% of their body weight at one year. Patients maintaining tirzepatide at the 2.5-mg starter dose for the same duration lost an average of 5.5% of their body weight at one year. Study leader Venky Soundararajan noted that these findings demonstrate a measurable pharmacologic effect well below the maintenance doses established in clinical trials, though they remain well short of the double-digit percentages seen at full doses.

By comparison, an earlier head-to-head trial demonstrated average one-year weight loss of 20.2% with tirzepatide and 13.7% with semaglutide at recommended higher doses. Many patients never achieve those maintenance doses due to cost, access, supply issues, tolerability, or individual treatment goals.

Comparing Adverse Events and Kidney Outcomes

Patients remaining on the initiation doses reported substantially less nausea and constipation than typically reported at higher manufacturer-recommended levels. However, the study recorded distinct side effect profiles between the two medications at these lower thresholds. According to Soundararajan, low-dose tirzepatide was associated with higher rates of kidney injury after two years, affecting 2.7% of patients compared to 0.4% with semaglutide.

Semaglutide currently holds U.S. approval to reduce the risk of worsening chronic renal disease, a protection not yet specifically approved for tirzepatide. Beyond renal outcomes, low-dose tirzepatide was linked to higher rates of:

  • Constipation (32.6% versus 22.4% for semaglutide)
  • Shortness of breath on exertion (9.6% versus 6.8%)
  • Muscle cramps (8.3% versus 4%)
  • Lumbar disc disease (3.3% versus 0.3%)

Conversely, patients on sustained low-dose semaglutide experienced higher rates of ankle swelling (6.9% versus 3.7% for tirzepatide), ear inflammation (5.6% versus 2.1%), and heavy sweating (5.5% versus 3.2%). Soundararajan cautioned that observational studies cannot prove direct cause and effect, emphasizing that patients not aiming for aggressive weight reduction deserve a clear understanding of these distinct adverse event profiles.

Alternative Maintenance Strategies Explored in Trials

Long-term weight maintenance remains a primary challenge in obesity treatment, as research shows individuals often regain, on average, 60% of lost weight within one year of stopping therapy entirely. Recent late-phase clinical trials sponsored by Eli Lilly and Company evaluated alternative maintenance strategies, such as transitioning patients from high-dose injectable medications to lower doses or oral formulations.

In the ATTAIN-MAINTAIN trial, adults who reached a weight plateau on high-dose injectable drugs switched to daily oral Foundayo or a placebo. Participants switching from high-dose Wegovy to daily oral Foundayo regained an average of 0.9 kilograms, or about 2 pounds, over one year. Those transitioning from high-dose Zepbound to Foundayo regained an average of 5 kilograms, or about 11 pounds, over the same period, a difference researchers attribute to the greater initial weight loss achieved with tirzepatide.

Dr. Mir Ali, a bariatric surgeon, bariatric medicine specialist and medical director of MemorialCare Surgical Weight Loss Center at Orange Coast Medical Center in Fountain Valley, California, noted that weight regain is common when patients discontinue therapy entirely, suggesting that continued treatment helps preserve reductions over time. Concurrently, Novo Nordisk has launched a clinical trial to evaluate lower maintenance doses of its Wegovy pill, though definitive results remain years away.
