Hepatitis C Elimination Efforts Lagging, Especially in Vulnerable Populations
Global efforts to eliminate hepatitis C as a public health threat by 2030, a goal established by the World Health Organization (WHO), are falling short, particularly among high-risk groups. A recent systematic review and meta-analysis of care cascades reveals significant disparities in screening, treatment access, and adherence across different populations and income levels.
Hepatitis C: A Global Health Challenge
Hepatitis C is a bloodborne viral infection that causes inflammation of the liver. It can lead to both acute and chronic illness, potentially resulting in serious complications like cirrhosis and liver cancer. According to the WHO, an estimated 50 million people worldwide have chronic hepatitis C, with approximately 1.0 million new infections occurring each year . In 2022, approximately 242,000 people died from hepatitis C-related complications .
Disparities in Care Cascades
The analysis highlighted significant gaps in the “care cascade” – the steps from diagnosis to cure – for key populations. These include:
- People living with HIV: Suboptimal screening rates and challenges in linking them to care.
- People who inject drugs: Limited access to screening and treatment, coupled with barriers to adherence.
- Men who have sex with men: Insufficient screening programs and disparities in treatment uptake.
- Incarcerated individuals: Poor screening coverage and difficulties in providing continuous care within correctional settings.
Regional Variations
The study found that high-income regions, particularly in Western Europe, demonstrated more promising results in hepatitis C elimination efforts. However, lower-income countries faced substantial service gaps and limited data availability, hindering progress.
Effective Treatments Exist, But Access is Limited
Direct-acting antiviral (DAA) medications offer a cure for over 95% of individuals with hepatitis C . Despite this high cure rate, access to diagnosis and treatment remains low globally. There is currently no effective vaccine available for hepatitis C .
The Importance of Testing and Treatment
Early detection and treatment are crucial for preventing serious liver damage and improving long-term health outcomes. The WHO recommends implementing triple testing for HIV, hepatitis B, and hepatitis C to improve diagnosis rates and facilitate integrated care . The WHO likewise published consolidated guidelines on HIV, viral hepatitis and STI prevention, diagnosis, treatment and care for key populations in 2022 .

Addressing the disparities in hepatitis C care cascades requires targeted interventions, increased funding, and improved access to testing and treatment, particularly for vulnerable populations. Continued monitoring and evaluation of elimination programs are essential to identify gaps and refine strategies to achieve the 2030 WHO goal.
