How Ozempic, Wegovy, and Mounjaro Affect Appetite and Digestion

Ozempic, Wegovy, Mounjaro, and Zepbound share common mechanisms of slowing digestion and suppressing appetite, yet their active ingredients, receptor targets, and approved medical uses differ significantly, according to nutrition and medical reporting by Basilino.

Active Ingredients and Receptor Targets

According to Basilino, Ozempic and Wegovy both utilize the active ingredient semaglutide, whereas Mounjaro and Zepbound rely on tirzepatide. Semaglutide acts exclusively on the GLP-1 receptor. In contrast, tirzepatide functions as a dual agonist targeting both GLP-1 and GIP receptors. This dual mechanism can produce a stronger appetite-suppressing effect for some patients, though individual responses vary widely, as reported by Basilino.

Approved Medical Uses and Regulatory Status

The U.S. Food and Drug Administration (FDA) approved Ozempic to treat type 2 diabetes and reduce specific cardiovascular risks in patients with type 2 diabetes. While doctors occasionally prescribe Ozempic off-label for weight management, its related product Wegovy holds specific FDA approval for chronic weight management in adults and children aged 12 years and older with obesity, or in adults with excess weight and at least one weight-related condition such as high blood pressure or high cholesterol.

Similarly, Mounjaro is approved for type 2 diabetes, while Zepbound is approved for weight management, both using the active ingredient tirzepatide, according to Basilino.

Dosage Protocols and Administration

Ozempic is administered as a once-weekly subcutaneous injection using a single-use pen. Treatment typically begins at a starting dose of 0.25 milligrams (mg) once weekly for four weeks, after which prescribing physicians may increase the dosage to 0.5 mg weekly. Depending on patient response and tolerability, dosages can be increased every four weeks up to a maximum dose of 2 mg per week for Ozempic pens, though off-label weight loss dosing plans may vary based on physician evaluation.

Side Effects and Dietary Approaches

All four injectable medications share a similar side-effect profile, including nausea, reduced appetite, slower stomach emptying, and constipation, which often manifest during initial weeks or following dose escalations, according to Basilino. Because these drugs delay gastric emptying, dietary strategies remain consistent across all brands. Basilino recommends consuming small, frequent protein-first meals while avoiding high-fat, greasy, or sugary foods that frequently trigger gastrointestinal discomfort.

The Oral Semaglutide Exception: Rybelsus

While the primary medications are administered via weekly injections where meal timing does not impact absorption, oral semaglutide tablets marketed under the brand name Rybelsus follow strict administration rules, according to Basilino. Citing the manufacturer’s patient information leaflet, Basilino notes that Rybelsus must be taken on an empty stomach after at least an 8-hour fast—typically first thing in the morning—with no more than a half-glass of water (up to 120 ml), at least 30 minutes before eating, drinking, or taking other oral medications. Tablets must not be split, crushed, or chewed.
