A recent study suggests that older adults who avoid meat may be somewhat less likely to reach age 100 than those who eat it. However, the findings are more nuanced than they initially appear and shouldn’t be interpreted as a simple condemnation of plant-based diets. The key lies in understanding the changing nutritional needs of the elderly and the importance of maintaining adequate body weight and muscle mass.

The Chinese Longitudinal Healthy Longevity Survey

Researchers followed more than 5,000 adults in China aged 80 or older who participated in the Chinese Longitudinal Healthy Longevity Survey (CLHLS), a nationally representative project initiated in 1998. By 2018, participants who did not eat meat were less likely to become centenarians compared to those who consumed meat. The CLHLS, as noted by the Center for Healthy Aging and Development Studies at Peking University, provides valuable longitudinal data on the health and lifestyle of the elderly in China.

Why the Apparent Contradiction?

At first glance, these findings seem to contradict decades of research linking vegetarian and plant-based diets to improved health outcomes, including lower risks of heart disease, stroke, type 2 diabetes, and obesity. Data from the CLHLS and other studies often attribute these benefits to higher fiber intake and reduced saturated fat consumption.

How Aging Changes Nutritional Needs

It’s crucial to consider the specific population studied – adults aged 80 and older – and how their dietary requirements differ from those of younger individuals. As people age, they experience significant physiological changes, including decreased energy expenditure and losses in muscle mass, bone density, and appetite. These changes increase the risk of malnutrition and frailty. Much of the existing evidence supporting the health benefits of meat-free diets comes from studies conducted on younger adults, not frail older populations.

Some research suggests that older adults who don’t eat meat may face a higher risk of fractures due to lower calcium and protein intake. In later life, the nutritional focus shifts from preventing long-term diseases to maintaining muscle mass, preventing weight loss, and ensuring that each meal provides sufficient nutrients.

The Importance of Body Weight

A critical detail from the study is that the lower likelihood of reaching 100 among non-meat eaters was only observed in underweight participants. No such association was found in older adults of healthy weight. Being underweight in older age is strongly linked to increased risks of frailty and mortality, suggesting that body weight is a key factor in these findings. This aligns with the “obesity paradox” in aging, where a slightly higher body weight is often associated with better survival in later life.

The Role of Animal-Source Foods

The reduced likelihood of reaching 100 among non-meat eaters was not observed in those who included fish, dairy, or eggs in their diets. These foods provide essential nutrients for maintaining muscle and bone health, including high-quality protein, vitamin B12, calcium, and vitamin D. Older adults consuming these foods were just as likely to live to 100 as meat-eaters. Researchers suggest that incorporating modest amounts of animal-source foods may help prevent undernutrition and loss of lean muscle mass in exceptionally old age compared to strictly plant-based diets.

Nutrition for Healthy Aging: A Personalized Approach

The broader takeaway is that there’s no one-size-fits-all diet for healthy aging. Nutrition should be tailored to an individual’s stage of life. Calorie needs generally decline with age, but the requirement for certain nutrients, such as protein, vitamin B12, calcium, and vitamin D, actually increases, particularly to preserve muscle mass and prevent frailty. Preventing malnutrition and weight loss often becomes more crucial than long-term chronic disease prevention in older adulthood.

Plant-based diets can still be healthy choices, but they may require careful planning and, in some cases, supplementation to ensure nutritional adequacy, especially in later life. Nutritional needs at 90 may differ substantially from those at 50. Dietary guidance should evolve over time, and adjusting eating patterns as you age is both expected and appropriate.
