HIV and Cardiovascular Disease: A Growing Concern
For decades, HIV infection was primarily understood as an immunological disease. However, with advancements in antiretroviral therapy (ART) leading to increased survival rates, a new challenge has emerged: a significant rise in cardiovascular disease (CVD) among people living with HIV (PLWH). Even with effective ART, PLWH continue to face a heightened risk of heart problems compared to their HIV-negative counterparts.
The Link Between HIV and Cardiovascular Disease
The increased incidence of atherosclerotic cardiovascular disease (ASCVD), heart failure and arrhythmias in PLWH is a complex issue driven by several interconnected factors. While ART has dramatically improved the lives of those with HIV, it doesn’t eliminate the risk of cardiovascular complications. Persistent immune activation and chronic inflammation, even in individuals on ART, are key contributors to this increased risk.
Inflammation: A Central Player
Chronic inflammation is now recognized as a primary driver of ASCVD in PLWH. Markers of inflammation, such as interleukin-6 (IL-6) and C-reactive protein, are often elevated in PLWH and are associated with increased mortality. Recent research suggests that this inflammation isn’t simply a byproduct of HIV infection but is influenced by unique immunological mechanisms.
Emerging Immunological Mechanisms
Several new insights are shedding light on the underlying causes of chronic inflammation in PLWH:
- Clonal Hematopoiesis: This involves the expansion of mutated blood cells, contributing to inflammation.
- Trained Immunity: This phenomenon suggests that the immune system “remembers” past encounters with pathogens, leading to an exaggerated inflammatory response. Microbial products and pro-inflammatory lipids likely play a role in initiating these pathways.
- Lipidomics: The study of lipids is revealing how altered lipid metabolism contributes to inflammation and cardiovascular risk.
Cardiac Manifestations in HIV
The spectrum of cardiac involvement in PLWH is broad, ranging from asymptomatic abnormalities to life-threatening conditions. Reported prevalence of cardiac involvement in individuals with AIDS has varied widely, from 28% to 73%.
Prevention and Management
Addressing cardiovascular risk in PLWH requires a multifaceted approach:
- Early ART Initiation: Prompt initiation of ART is crucial to suppress viral load and reduce immune activation.
- Aggressive Risk Factor Management: Controlling traditional cardiovascular risk factors like high blood pressure, high cholesterol, diabetes, and smoking is essential.
- Inflammation Monitoring: Regular monitoring of inflammatory markers may help identify individuals at higher risk.
- Cardiac Screening: Consideration of cardiac screening, particularly for those with multiple risk factors, may allow for early detection and intervention.
Looking Ahead
Research continues to deepen our understanding of the complex interplay between HIV, inflammation, and cardiovascular disease. Further studies are needed to identify novel therapeutic targets and develop effective strategies to reduce the burden of CVD in PLWH. As the population of PLWH continues to age, proactive cardiovascular care will be paramount to improving their long-term health and quality of life.
