Multiple sclerosis (MS) is a chronic, often disabling disease that affects the central nervous system – the brain and spinal cord. It’s one of the most common neurological disorders globally, impacting millions. While there’s currently no cure, significant advancements in understanding the disease and developing treatments are offering hope for those living with MS. This article provides an overview of MS, its progression, diagnostic criteria, and the evolving role of therapies like monomethyl fumarate.

What is Multiple Sclerosis?

Multiple sclerosis is an autoimmune disease where the immune system mistakenly attacks myelin, the protective sheath around nerve fibers. This damage disrupts communication between the brain and the rest of the body, leading to a wide range of symptoms. The cause of MS is not fully understood, but it’s believed to involve a combination of genetic predisposition and environmental factors. [1]

How is MS Diagnosed?

Diagnosing MS can be complex. Historically, the Atlas of MS outlined diagnostic criteria, which have been updated over time. The McDonald criteria, most recently revised in 2017 and again in 2024, are now used to diagnose MS. [1] These criteria focus on demonstrating evidence of disease dissemination in space (different areas of the central nervous system) and time (occurring at different points in time). Clinically isolated syndromes (CIS) – the first neurological symptoms suggestive of MS – are often evaluated for their potential to convert to definitive MS. [1]

Types of MS

MS presents in several forms:

  • Relapsing-Remitting MS (RRMS): The most common form, characterized by periods of new or worsening symptoms (relapses) followed by periods of recovery (remissions).
  • Primary Progressive MS (PPMS): A steadily worsening disease from the onset, without distinct relapses or remissions.
  • Secondary Progressive MS (SPMS): Initially resembling RRMS, but eventually transitioning to a progressive worsening of symptoms.
  • Clinically Isolated Syndrome (CIS): The first neurological event suggestive of MS. Not all individuals with CIS will develop MS.

Treatment Options for MS

Treatment for MS aims to modify the disease course, manage symptoms, and improve quality of life. Disease-modifying therapies (DMTs) are the cornerstone of MS treatment. These therapies work by reducing the frequency and severity of relapses, slowing disease progression, and reducing the formation of new lesions in the brain and spinal cord.

Monomethyl Fumarate (MMF)

Monomethyl fumarate (MMF), too known as Bafiertam, is an oral medication approved for the treatment of relapsing forms of MS. [3] It’s considered well-tolerated and safe, making it a valuable option for many patients. MMF’s exact mechanism of action isn’t fully understood, but it’s believed to have anti-inflammatory and neuroprotective effects. [3]

Other DMTs

Besides MMF, other DMTs available for MS include:

  • Interferon beta medications: Early treatment with interferon beta has been shown to impact conversion to definite MS. [1]
  • Glatiramer acetate: Another injectable medication that helps reduce relapse rates.
  • Teriflunomide: An oral medication that also reduces relapse rates. [2]
  • Cladribine: An oral medication used for highly active MS.
  • Ozanimod: A newer oral medication with demonstrated efficacy. [4]

The Rising Prevalence of MS

The prevalence of MS is increasing worldwide. The Atlas of MS estimates that approximately 2.9 million people globally are living with MS. [4] The reasons for this increase are not entirely clear, but may be related to improved diagnostic techniques, increased awareness, and potentially environmental factors.

Research into MS continues to advance, with ongoing efforts to develop new and more effective treatments. Understanding the underlying causes of MS and identifying biomarkers for early diagnosis and disease progression are key areas of focus.
