Roche’s Gazyva/Gazyvaro Shows Promise in Systemic Lupus Erythematosus Treatment

A phase III trial published in the Latest England Journal of Medicine demonstrates that Roche’s Gazyva/Gazyvaro (obinutuzumab), in combination with standard therapy, significantly reduces disease activity in adults with systemic lupus erythematosus (SLE). This marks a potential step forward in treating this potentially life-threatening autoimmune disease, which affects over three million people worldwide.

Key Trial Findings

The ALLEGORY trial, detailed in the March 5, 2026 issue of the New England Journal of Medicine, revealed that 76.7% of patients treated with Gazyva/Gazyvaro plus standard therapy achieved at least a four-point improvement in the SLE Responder Index 4 (SRI-4) at 52 weeks. This compares to 53.5% of patients receiving placebo plus standard therapy, representing a statistically significant difference of 23.1% (95% confidence interval: 12.5-33.6, p<0.001). Roche announced these findings on March 6, 2026.

Beyond the primary endpoint, Gazyva/Gazyvaro demonstrated superiority to placebo in all key and additional secondary endpoints. Notably, the median time to first flare – which can lead to permanent organ damage – was improved (hazard ratio: 0.58, 95% CI: 0.40-0.82, p=0.002). The remission rate also more than doubled with Gazyva/Gazyvaro (35.1% versus 13.8%, adjusted difference 21.2%, 95% CI: 11…).

Understanding Systemic Lupus Erythematosus (SLE)

Systemic lupus erythematosus (SLE) is a chronic autoimmune disease that can affect many different organs, including the skin, joints, kidneys, brain and other tissues. The immune system mistakenly attacks healthy tissues, causing inflammation and damage. Current treatments often focus on managing symptoms and preventing flares, but a more targeted approach has been needed.

Gazyva/Gazyvaro: A New Approach

Gazyva/Gazyvaro is a Type II anti-CD20 therapy that directly targets B cells, which play a key role in the inflammation and disease activity associated with SLE. If approved, it would be the first anti-CD20 therapy specifically for SLE to target B cells in this way.

Looking Ahead

The positive results from the ALLEGORY trial, published in the New England Journal of Medicine, suggest that Gazyva/Gazyvaro has the potential to become a new standard of care for individuals living with SLE. Further evaluation and potential regulatory approval will be crucial steps in making this treatment option available to patients. The data were also presented at the 15th European Lupus meeting, SLEuro 2026.
