The percentage of Americans classified as obese dropped from 40% to 37% between 2022 and 2025, according to a Gallup survey. This decline coincided with a sharp increase in the use of GLP-1 receptor agonists, a class of medication that mimics a natural hormone called glucagon-like peptide 1 to regulate blood sugar, control appetite, and slow digestion. While this weight reduction lowers the risk of heart disease, diabetes, and cancer, it introduces a significant physiological trade-off: approximately 25% of any weight lost comes from muscle mass, according to Dr. Caroline M. Apovian, co-director for Weight Management and Wellness at Harvard-affiliated Brigham and Women’s Hospital.

Why Rapid Weight Loss Accelerates Muscle Breakdown

When individuals reduce their calorie intake, exercise more, or use medications like GLP-1 receptor agonists, they experience a drop in total body mass that includes fat, muscle, and water. The body requires a calorie deficit to burn stored fat, but it also depletes glycogen reserves for energy. Once glycogen runs low, the body converts muscle protein into glucose as a quick backup energy source. Dr. Apovian notes that rapid weight loss, which frequently occurs with extreme low-calorie diets or GLP-1 therapies, accelerates this muscle-burning process and increases the overall rate of muscle mass depletion.

Three-Tiered Strategy to Minimize Muscle Loss

To protect lean muscle mass during weight reduction, health experts recommend a three-tiered approach focusing on resistance training, adequate protein consumption, and a slower rate of weight loss.

  • Resistance Training: Working against loads from weights, resistance bands, or body weight stimulates muscle growth and strength. Standard health guidelines recommend at least two weekly resistance training sessions alongside 150 minutes of moderate-intensity aerobic activity. Active muscles also consume calories during rest and recovery.
  • Increased Protein Intake: Dietary protein breaks down into amino acids necessary for building muscle. While general guidelines suggest 0.8 grams of protein per kilogram of body weight daily, research indicates adults aged 65 and older engaging in resistance training require 1.2 to 1.5 grams per kilogram, equating to roughly 90 to 112 grams daily for a 165-pound person. Dr. Apovian advises spreading protein intake across meals and snacks rather than consuming large amounts in a single sitting.
  • Gradual Weight Reduction: Physicians often start patients on lower doses of GLP-1 agonists to limit side effects and prevent excessively fast weight loss. Targeting a gradual reduction of 1 to 2 pounds per week helps protect muscle protein reserves.
