Electric bike crashes and related medical emergencies are surging, prompting municipalities to weigh new safety restrictions, helmet mandates, and infrastructure changes. According to a study published in the peer-reviewed journal Neurosurgery, trauma admissions involving pedal-powered and electric bikes and scooters accounted for nearly 7% of all trauma patients admitted to NYC Health + Hospitals/Bellevue in New York City.
Surge in Traumatic Brain and Spinal Injuries
Micromobility accidents are driving a heavy demand for acute medical care in major cities. According to research from NYU Langone Health, physical harm linked to electric bikes and scooters escalated sharply from less than 10% of trauma cases in 2018 to more than half of cases by 2023. One-third of the 914 patients treated for these incidents suffered a traumatic brain injury, with more than two-thirds requiring hospital admission and roughly 30% needing intensive care.
Collisions with motor vehicles represent the single largest danger to riders. According to the NYU Langone Health study, collisions with a car or truck caused about half of all recorded injuries. Furthermore, pedestrians struck by electric vehicles suffered brain injuries at nearly twice the rate of riders, based on an analysis of 69 pedestrian cases.
Protective gear usage remains low among riders involved in accidents. According to NYU Langone Health data, fewer than one-third of riders wore helmets, a factor linked directly to a significantly higher rate of brain and facial injuries. Additionally, 20% of patients tested positive for alcohol, which researchers associated with more severe head trauma and even lower rates of helmet compliance.
“We are seeing more and more of these injuries firsthand,” Hannah Weiss, a resident in the Department of Neurosurgery at the NYU Grossman School of Medicine and corresponding author of the study, said in a public statement. “The data point to actionable solutions—helmet use, safer bike lane design and enforcement—that could prevent many of these injuries and better protect both riders and pedestrians.”
Local Authorities Confront Class 3 E-Bike Speed and Weight
Metropolitan areas are grappling with the practical realities of high-speed micromobility on shared streets and sidewalks. In the Detroit, Michigan, metro area, local police report an uptick in e-bike crashes while emergency room physicians treat a rising volume of trauma patients, prompting discussions in some communities regarding potential bans or strict local ordinances.
Class 3 e-bikes are capable of reaching speeds up to 28 miles per hour, blurring the line between traditional bicycles and motorized vehicles. Robert Sherwin, an emergency room doctor at the Detroit Medical Center, told Fox 2 Detroit that these machines present unique safety challenges for operators. “These Class 3 bikes are probably more like motor vehicles,” Sherwin said. “They travel up to 28 miles an hour, and a lot of riders aren’t expecting that, might not be experienced in that. These e-bikes are heavier than regular bikes, and they travel much faster, but the human body doesn’t change just because a bike has a motor.”
Street safety concerns extend to interactions with automobile traffic. John Hughes, a local bike shop owner, told Fox 2 Detroit that the core hazard involves sharing congested corridors with distracted drivers. “Cars aren’t looking out for bikes… people don’t respect bikes as a form of transportation, and that’s where it gets dangerous,” Hughes said, adding that constant cellphone use among motorists exacerbates the risk.
State Legislation and Regulatory Responses
In New Jersey, a state law enacted earlier this year mandates that e-bikes must be registered, and operators must be at least 15 years old while possessing either a valid driver’s license or a specific e-bike license. Supporters of the New Jersey statute argued that licensing, helmet mandates, and registration protocols are necessary to promote responsible riding habits and minimize severe bodily harm following fatal crashes involving teenagers.
Public health experts emphasize that physical infrastructure must evolve alongside transportation trends. Paul P. Huang, associate professor in the Department of Neurosurgery at NYU Grossman School of Medicine and chief of neurosurgery at NYC Health + Hospitals/Bellevue, noted that urban planning must adapt. “Future studies should track these injuries across multiple cities and measure whether protected bike lanes, helmet programs and speed enforcement actually reduce the number of brain and spine surgeries we perform,” Huang stated.
