A Liquid Alternative to the Dental Drill

Silver diamine fluoride at 38% stops tooth decay progression in more than half of treated primary teeth among children under six without drilling or anesthesia, according to a study published in JAMA Pediatrics by researchers at the University of Michigan.

Tooth decay affects more than 40% of children in the United States, according to the University of Michigan research team. Severe early childhood caries creates significant health and economic burdens. Traditional dental treatment typically involves removing damaged tooth structure and placing a filling.

To bypass the need for dental drills, injections, and sedation, the research team evaluated the effectiveness of 38% silver diamine fluoride, an inexpensive liquid already utilized by dentists to treat tooth sensitivity.

Clinical Trial Design and Application

The clinical trial recruited 830 children under the age of six from dental and pediatric practices. Participants suffered from severe early childhood caries with active cavitated carious lesions.

Researchers applied either 38% silver diamine fluoride or a placebo to the lesions at the initial visit and again six months later. Dentists brushed the liquid directly onto the decay using a small foam-tipped applicator. The procedure requires no alteration of the tooth structure and takes only seconds per tooth.

Efficacy Rates and Permanent Staining

Results published in JAMA Pediatrics show that 38% silver diamine fluoride successfully halted 57,5 %, 54 % and 50,2 % of active cavitated lesions on primary teeth at three months, 6 months and 8 months, respectively, outperforming the placebo group.

Fontana noted that the treatment carries a distinct visible drawback: the silver in the formula permanently stains the decayed portion of the tooth dark.

Safety Profile and Patient Demographics

Adverse effects were reported by 196 young participants in the treatment group and 180 in the placebo group. Most side effects proved mild to moderate and appeared at similar rates across both cohorts.

The research team suggests the treatment holds particular value for very young children, older adults, individuals with physical or developmental disabilities, and patients experiencing severe dental anxiety. The intervention also benefits people with limited access to conventional dental care or those unable to tolerate standard procedures.

Potential for FDA Approval

Fontana stated that broader acceptance could treat many cavities before they cause pain, lead to infections, or require surgical intervention.

The study also provides evidence that may encourage the Food and Drug Administration to approve 38% silver diamine fluoride specifically as a treatment for dental caries.
