How Oral Semaglutide Works and Differs from Injections

Semaglutide mimics the natural hormone GLP-1 to regulate hunger, satiety, and food intake, according to the EMA. Dr. Ulrike Thieme, medical director at the telemedical provider ZAVA, explains that the medication does more than simply suppress appetite. It slows gastric emptying, particularly at the start of treatment, and supports blood sugar regulation by increasing insulin secretion and lowering glucagon when blood glucose is elevated, as noted by ZAVA.

The transition from a weekly injection to a daily pill involves a significant change in dosage figures. According to the EMA, the oral medication is available in strengths of 1.5, 4, 9, and 25 milligrams.

Administration rules for the tablet differ strictly from the injection schedule. Side Effects and Health Risks

Because both formulations contain the same active ingredient, the fundamental side effect profile remains comparable, according to Dr. Thieme. Gastrointestinal issues represent the most frequent adverse events, including nausea, vomiting, diarrhea, constipation, abdominal pain, bloating, and reflux. Data from the OASIS-4 trial showed that 74 percent of patients on oral semaglutide experienced gastrointestinal side effects, compared to 42.2 percent in the placebo group. Dr. Thieme clarifies that most of these reactions were mild to moderate rather than severe.

Less common but medically significant risks include pancreatitis, gallstones, gallbladder inflammation, severe gastrointestinal complications, allergic reactions, and acute kidney injury resulting from severe dehydration caused by vomiting or diarrhea. For patients with type 2 diabetes, hypoglycemia is a risk when the drug is combined with insulin or specific blood-sugar-lowering medications, and diabetic retinopathy can worsen in susceptible individuals, according to ZAVA.

Insurance Coverage and Long-Term Management

Statutory health insurance in Germany classifies semaglutide used strictly for weight loss as a lifestyle medication under Section 34 of the Social Code Book V (SGB V), meaning insurance generally does not cover the cost, according to Martin Schulze. Patients must pay for the prescription treatment out of pocket, which Schulze notes places added pressure on lawmakers to reevaluate policies surrounding chronic obesity treatment.

Experts emphasize that medication alone is insufficient to maintain weight loss long-term.
