According to a major study published on September 8, 2026, in JAMA Neurology involving more than 500,000 participants, researchers discovered that non-smokers with higher concentrations of exhaled carbon monoxide exhibited a lower risk of developing Parkinson’s disease. This finding offers a biological clue as to why cigarette and tobacco consumers historically show lower rates of the neurodegenerative disorder than the general population.

### Carbon Monoxide Levels and Parkinson’s Risk

The international research team, drawing on data from investigators at the University of Oxford as well as institutions in Tongxiang and Beijing, found a dose-dependent association among people who had never smoked. Individuals with exhaled carbon monoxide levels measuring 3 parts per million (ppm) or higher demonstrated roughly a 30 percent reduction in their risk of developing Parkinson’s disease.

Dr. Michael Okun, a neurology professor at the University of Florida, noted in an interview with Newsweek that while high concentrations of carbon monoxide act as a poison, the human body naturally produces small amounts of the gas to serve as a signal molecule. Smokers naturally maintain higher exhaled carbon monoxide levels due to cigarette smoke exposure, which raises questions about whether these specific biological signaling pathways might protect vulnerable, dopamine-producing brain cells from oxidative stress and inflammation.

### Decades of Research on Tobacco and Brain Health

The new JAMA Neurology findings build upon a substantial body of prior epidemiological research identifying an inverse relationship between tobacco use and Parkinson’s incidence. A 65-year observational study of more than 30,000 British doctors, published in 2020 in the journal Neurology, established that smokers possessed a 30 to 40 percent lower risk of developing Parkinson’s disease compared to people who had never smoked.

An earlier twin study published in Neurology in 2002 similarly observed that among identical twins, the sibling free of Parkinson’s tended to smoke more than the sibling affected by the condition. Despite these consistent statistical patterns, public health agencies emphasize that tobacco consumption remains a leading cause of preventable illness. According to the Centers for Disease Control and Prevention (CDC), smoking and secondhand smoke cause more than 480,000 deaths annually in the United States and contributes to cancers, heart disease, stroke, lung diseases, and type-2 diabetes.

### Exploring Alternative Protective Compounds

While carbon monoxide presents a compelling mechanistic explanation, other experts point to alternative components within the tobacco plant. Research examining smokeless tobacco products—such as Swedish snus, where users absorb nicotine through the oral mucosa—has yielded comparable risk reductions. A study published in 2016 in the American Journal of Epidemiology found that users of Swedish snus also experienced a lower incidence of Parkinson’s disease.

Dr. Alberto Ascherio, a professor of medicine at the Harvard Medical School who was involved in a 2005 study on smokeless tobacco published in Wiley InterScience, told Newsweek he suspects specific compounds within tobacco leaves offer neuroprotective properties. These may include inhibitors of Monoamineoxidase B (MAO-B), an enzyme that breaks down brain neurotransmitters like dopamine. By inhibiting MAO-B, therapeutic agents can help maintain higher dopamine availability, which is critical since Parkinson’s symptoms emerge when dopamine-producing neurons degrade and die.

### The Role of Nicotine and Future Directions

Additional scientific investigations focus on nicotine itself. According to Dr. Okun, nicotine remains a primary candidate because it interacts directly with nicotinic acetylcholine receptors in brain networks governing movement, potentially mitigating inflammation and supporting mitochondrial survival in dopaminergic cells.

Parkinson’s disease is the second most common neurodegenerative disorder in the United States after Alzheimer’s disease. Projections from the nonprofit Parkinson’s Foundation estimate that the number of people living with Parkinson’s in the US will rise to 1.2 million by 2030.

Researchers stress that the statistical correlations involving carbon monoxide or tobacco components do not endorse smoking as a preventive measure. Instead, scientists aim to isolate the exact biological mechanisms responsible for the protective signal so that safe, targeted therapies can be developed to prevent or slow the progression of Parkinson’s disease without exposing patients to the severe health hazards of tobacco.
