The breakthrough procedure preserved Hibbert’s eyesight, which was actively threatened by an 11-millimeter non-cancerous tumor pressing against his optic nerves and carotid arteries.

How Live AI Assisted the London Brain Surgery

During the operation performed at UCLH, surgeons removed the pituitary gland tumor by inserting an endoscope through the patient’s nose to the base of the skull. To navigate the delicate anatomical landscape, a bespoke AI tool analyzed a live video feed of the procedure on a secondary screen in real-time. According to BBC Health reporter Smitha Mundasad, the system tracked surgical instruments and highlighted regions where critical nerves and blood vessels were concealed behind bone and membrane.

Researchers at University College London developed the technology, with financing from the National Institute for Health and Care Research and Google. Professor Hani Marcus, who helped carry out the procedure, stated that the system is trained on more operations than most surgeons witness in a lifetime, functioning as an expert second pair of eyes to help clinical teams avoid critical vessels and nerves.

Diagnosis and Patient Journey

Until eighteen months prior to the operation, Hibbert led an active lifestyle that included a daily two-mile walk at lunchtime. The condition was discovered after he lost consciousness and suffered a seizure in the road during one of these walks. Subsequent medical scans identified the pituitary tumor, a marble-sized organ located at the base of the skull responsible for secreting vital hormones that regulate growth, metabolism, blood pressure, and temperature.

As the growth expanded, it narrowed Hibbert’s peripheral vision and caused severe fatigue and dizziness. Given that complication rates for pituitary tumor removal typically include a 25 to 50 percent chance of leaving residual tumor tissue and a 0.5 to 2 percent risk of major blood vessel injury, Hibbert agreed to participate in the research trial. “If patients are not prepared to join research how can doctors ever learn and how can medicine ever progress?” Hibbert said, as reported by the BBC.

Clinical Implications and Future Operations

The successful outcome highlights the potential for real-time AI tools to enhance patient safety during complex neurosurgical procedures where internal anatomy varies significantly between individuals. Standard operations typically rely on static scans captured prior to surgery, whereas live AI analysis offers continuous anatomical mapping akin to facial recognition software adapted for internal surgical structures. UCLH researchers plan to continue evaluating the technology to refine its utility in future clinical applications.
