High blood levels of the popular artificial sweetener xylitol are associated with a significantly increased risk of major adverse cardiovascular events, including heart attack and stroke, according to a study presented at the ESC Cardiology Congress in Germany. Researchers following thousands of participants over multi-year periods found that elevated xylitol concentrations in the body tracked with a higher incidence of cardiovascular events and cardiovascular mortality.

Study Findings and Patient Cohorts

The research evaluated two large participant groups from Canada and Britain, totaling more than 17,700 individuals, according to data presented at the cardiology conference. Over a six-year follow-up period, participants in the Canadian Longitudinal Study on Aging who had the highest blood levels of xylitol faced a 57% greater risk of death, myocardial infarction, or stroke compared to those with the lowest levels. A separate 30-year analysis of the European Prospective Investigation into Cancer (EPIC)-Norfolk group revealed an 18% greater risk of major adverse cardiovascular events (MACE) among participants with the highest xylitol concentrations.

Both cohorts demonstrated a dose-dependent association, meaning higher blood xylitol concentrations corresponded directly with increased rates of cardiovascular events. These elevated risks persisted even after researchers adjusted for traditional confounding factors such as age, gender, body mass index, hypertension, diabetes, and lipid levels.

Mechanisms and Clinical Perspective

Xylitol is a sugar alcohol commonly used as a low-calorie alternative to sugar in foods, drinks, chewing gum, and dental care products. Previous research cited by the study presenters indicates that xylitol can enhance the ability of blood platelets to form clots. Dr. Marco Witkowski of Charité University Hospital in Berlin, Germany, noted that artificial sweeteners are generally regarded as safe by regulatory agencies and are consumed by people seeking healthier alternatives to sugar. However, he emphasized that these long-term findings highlight gaps in understanding the cardiovascular safety of xylitol and indicate that further studies are warranted as product usage increases.

Dr. Aseem Malhotra, a London-based cardiologist and public health campaigner who was not involved in the research, told Fox News Digital that the findings should be taken very seriously given the plausible mechanism linking xylitol to increased cardiovascular risk. Malhotra noted that he frequently observes patients experiencing anxiety and palpitations that resolve after discontinuing artificial sweeteners, advising patients to avoid processed sugars and artificial sweeteners to break the addiction.

Study Limitations and Natural Production

Study authors noted that the research was observational and does not definitively prove that dietary xylitol causes cardiovascular events, underscoring the need for further clinical confirmation. Furthermore, the report does not establish whether high blood levels of xylitol necessarily reflect direct dietary consumption alone. The sugar alcohol can also be produced naturally within the human body as part of glucose metabolism, complicating direct measurements of dietary intake versus endogenous production.
