Stopping GLP-1 Medications Can Quickly Reverse Heart Benefits
For the roughly one in eight U.S. Adults taking GLP-1 receptor agonists like Ozempic and Wegovy, discontinuing these medications—even for a short period—can lead to a concerning reversal of cardiovascular benefits and an increased risk of serious heart events. New research highlights the importance of consistent use for those seeking to protect their heart health.
The Cardiovascular Risks of Discontinuation
A study published in BMJ Medicine followed over 333,000 adults with Type 2 diabetes for three years, comparing those taking GLP-1 medications to those using sulfonylureas. The findings revealed a significant correlation between consistent GLP-1 use and reduced cardiovascular risk. Patients who remained on GLP-1s throughout the study period experienced an 18% reduction in the risk of heart attack, stroke, and death.
However, this protective effect began to erode with even brief interruptions. Quitting GLP-1s for as little as six months resulted in a 4% increase in cardiovascular risk compared to continued use. A two-year gap in treatment pushed that risk up to 22% 1. Notably, the study found that it takes less time to undo the cardiovascular benefits than it does to build them.
How GLP-1s Protect the Heart
GLP-1 medications offer multiple benefits that contribute to improved cardiovascular health. Beyond weight loss, these drugs can reduce blood pressure, lower cholesterol, improve insulin resistance, and decrease inflammation 1. These combined effects create a protective environment for the heart.
Dr. Ziyad Al-Aly, a clinical epidemiologist at WashU Medicine and the chief of the research and development service at the VA Saint Louis Health Care System, describes the reversal of these benefits as a “metabolic reversal.” Stopping the medication can lead to weight regain, increased cholesterol levels, and a spike in markers of inflammation, all of which can strain the cardiovascular system 3.
Restarting GLP-1s Doesn’t Fully Restore Protection
The study also indicated that simply restarting a GLP-1 medication after a period of discontinuation doesn’t necessarily restore the full level of cardiovascular protection achieved with consistent use. The reasons for this are not fully understood, but researchers suggest that the body may struggle to fully recover from the “whiplash” effect of repeated interruptions 3.
Cost and Access as Barriers to Consistent Use
A significant reason people discontinue GLP-1 medications is the high cost and lack of insurance coverage. The financial burden—potentially $400 per month—can be prohibitive for many patients 2.
Recommendations for GLP-1 Users
Dr. Al-Aly emphasizes the importance of discussing treatment goals and the feasibility of long-term medication adherence with a healthcare provider before starting a GLP-1 medication. If a patient anticipates difficulty continuing the medication, exploring options like lower maintenance doses or discussing potential alternatives is crucial. Complete discontinuation is likely to undo the benefits gained 2.
As new and potentially more affordable GLP-1 medications are developed, access to these life-improving treatments may become more widespread.
