A new study published in JAMA Ophthalmology reveals that struggling to read more than six lines on a Pelli-Robson contrast sensitivity chart may serve as an early visual warning sign for older adults. According to researchers at the University of Michigan and the University of Michigan’s Institute for Social Research, this specific threshold identifies everyday vision problems that standard, high-contrast eye exams frequently fail to detect.

Limitations of Traditional Eye Charts in Aging Adults

Traditional eye examinations rely primarily on high-contrast black letters on a white background to measure visual acuity. According to University of Michigan neuro-ophthalmologist Lindsey De Lott, these standard tests often miss important vision changes that occur with age. Instead of sharp black-and-white distinctions, researchers measure contrast sensitivity using letters in varying shades of gray that become progressively lighter and harder to discern.

While medical professionals have long understood that contrast sensitivity effectively detects certain visual impairments, previous clinical practice lacked a concrete numerical threshold. According to De Lott, doctors previously had no reliable baseline indicating where contrast sensitivity declines begin interfering with day-to-day activities like recognizing faces, reading print, or watching television.

Connecting Contrast Sensitivity to Daily Life

The study analyzed data from 4,475 Medicare beneficiaries drawn from the National Health and Aging Trends Study, making the findings nationally representative. According to Shu Xu, a postdoctoral fellow at the University of Michigan’s Institute for Social Research, prior definitions of visual “yellow lights” relied solely on patients reporting no visual problems. By establishing a functional anchor, researchers connected specific test scores directly to older adults’ real-world experiences.

Many older adults report that their vision is less reliable in specific environments, such as navigating uneven sidewalks at dusk, reading medication labels under dim light, or distinguishing objects that blend into their background. According to the research team, individuals began reporting these everyday difficulties even when their visual acuity scores on traditional charts would not be considered impaired.

Implications for Clinical Care and Public Health

Poor contrast sensitivity correlates closely with real-world safety and functional outcomes, including an increased risk of falls, driving difficulties, and loss of independence. Traditional clinical scoring methods typically label scores two standard deviations below the mean as impaired, which only indicates that a patient falls below average rather than predicting specific functional trouble.

Establishing this new Pelli-Robson chart threshold allows clinicians to intervene before patients reach a critical stage of visual decline. According to the study authors, adults aged 65 and older who report visual struggles despite passing routine exams may benefit specifically from contrast sensitivity evaluations. Researchers are currently tracking whether older adults scoring below this threshold face elevated risks of future declines in overall health and autonomy.
