Platelet transfusions administered to intensive care unit patients with sepsis and thrombocytopenia are associated with a significantly increased risk of 28-day and 90-day all-cause mortality, according to retrospective database findings published in Frontiers. The analysis, which utilized data from the Medical Information Mart for Intensive Care III database, raises new questions about standard transfusion practices for critically ill patients.

Mortality Risks Linked to Platelet Transfusion

According to the Frontiers study, researchers evaluated sepsis patients with platelet counts of 150,000/μL or lower. Among 7,765 eligible patients, 677 received platelet transfusions and were matched using propensity score matching with 677 patients who did not receive transfusions. The data showed that 28-day all-cause mortality reached 36.9 percent in the transfusion group, compared to 30.4 percent in the control group, yielding an odds ratio of 1.21. Furthermore, 90-day all-cause mortality climbed to 50.8 percent for transfused patients, versus 44.6 percent for non-transfused patients.

Impact on ICU Stay and Hospital-Free Days

Beyond mortality rates, the study tracked secondary outcomes including intensive care unit stays and red blood cell transfusion requirements. Patients who received platelet transfusions experienced fewer mean 28-day ICU-free days, averaging 15.88 days compared to 18.64 days for the control group. Hospital-free days similarly dropped to 10.29 days for transfused patients, down from 11.43 days. Red blood cell transfusion rates showed no statistically significant difference between the two cohorts, according to Frontiers.

Current Clinical Guidelines and Uncertainty

Sepsis affects an estimated 15 million people annually and remains a major global public health challenge, with thrombocytopenia occurring in roughly 55 percent of sepsis cases. While platelet transfusions are common in oncology and prior to invasive procedures, evidence supporting their efficacy in sepsis remains scarce and low in quality. The Surviving Sepsis Campaign guidelines address thrombocytopenia management in vague terms, and apart from expert opinion and single-center retrospective studies, robust data on clinical outcomes like bleeding and death have been largely absent, as noted by Frontiers.
