Texas officials and hospital systems face a potential loss of $27 million a day in federal Medicaid matching funds, threatening care access for roughly four million low-income residents, according to the Texas Hospital Association. The funding impasse stems from a dispute between the Trump administration and state leaders over local hospital tax structures and Medicaid compliance rules.

Medicaid Funding Dispute and the CHIRP Shortfall

At the center of the standoff is about $9.8 billion in withheld federal funds for the upcoming year, primarily affecting the Comprehensive Hospital Increase Reimbursement Program (CHIRP). According to the Texas Hospital Association, local governmental entities collect roughly $4 billion annually in taxes from hospitals. The federal government matches these funds under CHIRP to help bridge the gap between outdated state Medicaid payment rates and the actual costs hospitals incur when treating Medicaid patients.

New federal Medicaid rules took effect September 1, compounding financial pressures on facilities that were already bracing for a 10% funding cut. Sara González, a vice president of advocacy, public policy, and political strategy for the Texas Hospital Association, stated that facilities cannot absorb massive losses on their Medicaid portfolios without cutting services across the board for all patients, regardless of insurance status. John Hawkins of the Texas Hospital Association warned that if the standoff persists for another month, certain North Texas hospitals could face employee layoffs and furloughs.

Federal Scrutiny and State Pushback

State documents indicate the Centers for Medicare & Medicaid Services is withholding payments due to questions surrounding local tax collection methods and how hospitals calculate patient spending under new federal requirements. Federal officials are also seeking additional evidence that Texas enforces its state law barring noncitizen adults from receiving Medicaid services.

Abbott asserted that the tax structures enacted by the Texas Legislature and implemented by local governments comply fully with federal law. Robert Fries, chief financial officer for Children’s Health in Dallas, noted in a public statement that the funding uncertainty directly threatens pediatric specialty care, behavioral health services, and the specialized workforce required to maintain the state’s healthcare safety net.
