Most Appendix Cancer Patients Can Safely Skip Chemotherapy, Study Finds

Most patients diagnosed with localized appendix cancer can safely skip chemotherapy following surgery, according to a clinical study published in JAMA Surgery. Researchers at The University of Texas MD Anderson Cancer Center found that fewer than 10% of patients experienced a cancer recurrence after surgical resection, and postoperative chemotherapy failed to show any association with improved survival or a lower risk of relapse.

Understanding Localized Appendix Cancer Recurrence Rates

The retrospective analysis reviewed clinical outcomes for 439 patients diagnosed with stage 1 through stage 3 appendix cancer who received treatment at MD Anderson between 2000 and 2024. Among that total population, researchers tracked cancer recurrence across 202 patients who underwent surgical intervention. During a median follow-up period exceeding five years, just 19 patients—representing 9.4% of the surgical cohort—experienced a recurrence, according to the MD Anderson data.

Recurrence rates varied by initial disease staging. Only 6% of patients with stage 2 disease experienced a return of the cancer, while 19.5% of patients with stage 3 disease faced a relapse. Patients who experienced recurrence faced poorer long-term outcomes, registering a five-year overall survival rate of 77.2%. In contrast, patients who did not experience a recurrence achieved a five-year overall survival rate of 95.7%, as reported by the study authors.

Challenging Colorectal Treatment Parallels

Historically, clinicians treated appendiceal tumors using protocols borrowed from colorectal cancer due to the rarity of the disease. John Paul Shen, M.D., associate professor of Gastrointestinal Medical Oncology at MD Anderson and co-leader of the analysis, stated that an outdated notion dictating appendix cancer behaves similarly to colon cancer has driven treatment decisions for too long. The study findings establish that appendix cancer possesses a distinct biology, meaning the risk of relapse remains low for most patients with localized disease, according to Shen.

The research team concluded that postoperative chemotherapy provided no survival benefit or reduction in relapse risk, even among patient subgroups deemed to be at higher risk. Sacha El Khoury, M.D., a research fellow in Gastrointestinal Medical Oncology at MD Anderson, noted that appendiceal cancers are not all the same, emphasizing that combining tumor histology with molecular features allows clinicians to better pinpoint true high-risk patients.

Genetic Mutations and Tumor Histology

To pinpoint which patients face a higher probability of relapse, researchers analyzed specific tumor characteristics and genomic alterations. The study identified that distinct genetic changes correlate with a greater risk of cancer returning depending on the tumor type. Specifically, TP53 mutations linked to recurrence in goblet cell tumors, whereas GNAS mutations associated with a higher relapse risk in non-goblet tumors.

Funding for the research came from the Cancer Prevention and Research Institute of Texas (CPRIT), the Appendix Cancer Pseudomyxoma Peritonei Research Foundation, and Conquer Cancer, the ASCO Foundation. The authors noted that study limitations include its single-center design at MD Anderson, meaning broader prospective research is necessary to validate these findings across wider patient populations.
