Google is rolling out Google Health version 5.09 for Android and iOS, introducing new Health Guardian tracking capabilities for Pixel Watch users alongside expanded workout syncing and fitness logging updates. The update brings long-term trend analysis for blood pressure, insulin resistance, and sleep breathing quality directly to compatible wearables, including the Pixel Watch 3, 4, and 5.

Health Guardian Trends and Monitoring Requirements

The new Health Guardian features give users visibility into physiological shifts that impact cardiovascular and metabolic wellness. To unlock these insights, specific wear-time thresholds must be met during September:

  • Blood Pressure Trends: Available in the Heart section of the Health tab, this feature tracks blood pressure patterns before they place strain on the heart. It requires users to wear their watch for five or more consecutive days in September.
  • Insulin Resistance Trends: Located in the Metabolic section, these trends populate for users who have worn their watch for seven or more days and nights over the same period.
  • Sleep Breathing Quality: Housed in the Sleep tab and Respiratory section, these metrics track daily and monthly breathing patterns to show how respiration affects daily energy levels.

Google states that the first monthly Health Guardian summary will appear at the beginning of October, with ongoing data accessible within the app’s Health tab.

Cardio Load Expands to Third-Party Wearables and Apps

Google Health 5.09 broadens its ecosystem integration by allowing workouts tracked in external apps and devices to count toward a user’s Cardio Load. As long as the alternative app or device—such as a Garmin or Apple Watch—tracks and syncs heart rate data during a workout, that session now contributes to the user’s overall training metrics.

According to Google Health documentation, Cardio Load is calculated using the TRIMP (TRaining IMPulse) model, which evaluates heart rate during activity alongside age, resting heart rate, and sex. For users with a Google Health Premium subscription who opt into the Google Health Coach, target loads dynamically adjust based on weekly training focus—categorized as Recovery, Maintain, or Build. Users without a Premium subscription continue to have their weekly targets determined by their average cardio load over the preceding four-week period.

Nutrition and Fitness Management Updates

The release also addresses workflow friction in nutrition tracking and activity logging. Custom food creation on iOS features streamlined toggling for serving units and meal times, while a bug that caused deleted items to persist in custom food lists has been resolved.

On the fitness front, the Fitness tab now provides expanded options for logging completed activities via an updated “Log activity” button. Google also reports that an issue causing missing historical step streaks has been fixed, and affected user data is being restored.
