Atrogi’s ATR-258: Pioneering a Novel Approach to Metabolic and Muscle Health

In the pursuit of innovative weight loss and metabolic health solutions, Atrogi AB, a clinical-stage biotech company, is making waves with its lead candidate, ATR-258. This first-in-class oral therapy aims to mimic the effects of exercise, offering a potential new treatment for muscle-sparing weight loss and a range of metabolic disorders. The company is focused on developing therapies that harness skeletal muscle signaling to treat metabolic diseases.

What is ATR-258?

ATR-258 is a novel GPCR pathway-signalling modulator designed to transform metabolic and muscle health. It functions as a GRK-selective long-acting β2-agonist, selectively targeting the β2-adrenergic receptor signaling pathway. This pathway plays a crucial role in regulating muscle physiology and metabolism. According to Atrogi, ATR-258 drives fat loss, increases muscle mass, and improves metabolism – essentially replicating the benefits of physical exercise.

Clinical Trial Progress

As of March 18, 2026, Atrogi announced that the first subjects have been dosed in a human trial evaluating the muscle physiological effects of ATR-258. This 8-week, investigator-initiated, interventional study is being led by Associate Professor Morten Hostrup at the University of Copenhagen, a leading authority on β2-adrenergic receptor signaling in skeletal muscle. The trial involves overweight male volunteers receiving daily oral doses of ATR-258.

The study aims to determine the extent to which ATR-258 replicates the muscle physiological effects of traditional β2 agonists and its potential application in various muscle atrophic conditions.

The Science Behind ATR-258

Atrogi’s breakthrough technology, validated by research published in Cell in June 2025, enables the development of next-generation, highly selective β2-agonists for chronic use. This selectivity is key, as it aims to minimize the side effects often associated with earlier β2-agonists. The company is pioneering first-in-class treatments to revolutionize metabolic health and redefine the management of muscular wasting and diabetes.

Potential Applications

ATR-258 holds promise as a novel treatment for a variety of conditions, including:

  • Muscle-sparing weight loss
  • Muscle wasting (atrophy)
  • Metabolic disorders
  • Conditions associated with immobilization, aging, and weight loss
