Doctor Sentenced to 15 Years for $45 Million Botox Fraud Scheme

A California doctor was sentenced to 15 years in federal prison for orchestrating a massive $45 million Medicare fraud scheme centered around medically unnecessary and phantom Botox injections, the Department of Justice reported. Violetta Mailyan, 45, of Glendale, operated Healthy Way Medical Center, a clinic that billed federal health insurance for thousands of treatments that were never performed or were administered strictly for cosmetic purposes without proper primary care referrals.

Luxury Purchases and Bogus Clinic Hours

Evidence presented at trial showed that Mailyan used the stolen taxpayer funds to finance an extravagant lifestyle, purchasing two Tesla vehicles—including a Cybertruck—along with five pieces of California real estate, a $12,000 17th-century crossbow, and a $3,000 painting, according to the New York Post. Court documents revealed that more than $19 million in fraudulent claims involved Botox injections purportedly administered on days when her clinic was completely closed.

Prosecutors demonstrated that Mailyan continued submitting high-volume insurance claims while traveling across the country and internationally. In one instance highlighted by investigators, she billed Medicare more than $200,000 for treatments supposedly provided to patients in California while she was actually relaxing on a beach in Cabo, Mexico.

Obstruction and Restitution Orders

When federal investigators began closing in on her operation, Mailyan attempted to cover her tracks by falsifying medical records and forging beneficiary signatures, including submitting paperwork for a man who was incarcerated in a federal prison at the time of the alleged treatment. Following a six-day jury trial that ended in May 2026, a jury convicted Mailyan of nine counts of wire fraud and three counts of obstruction of a criminal investigation into a health care offense.

In sentencing Mailyan, the presiding judge condemned the operation as truly breathtaking in scope and shocking in its duration. Alongside her 15-year prison sentence, the court ordered the immediate forfeiture of more than $16 million in seized real estate, bank funds, and personal property, and mandated that Mailyan pay $32,904,049.41 in direct restitution to Medicare.
