Ivonescimab Shows Promise in Squamous Non-Slight Cell Lung Cancer Treatment
A modern study, HARMONi-6, reveals that ivonescimab combined with chemotherapy demonstrates improved progression-free survival compared to tislelizumab plus chemotherapy as a first-line treatment for advanced squamous non-small-cell lung cancer (NSCLC). However, the benefits appear to be significantly influenced by PD-L1 expression levels in the tumor.
Understanding the HARMONi-6 Trial
The HARMONi-6 trial was a randomized, double-blind, phase 3 clinical trial conducted across 50 sites in China [1]. Researchers enrolled 500 patients aged 18 to 75 with previously untreated, unresectable stage IIIB, IIIC, or stage IV squamous NSCLC and a excellent performance status (Eastern Cooperative Oncology Group score of 0 or 1). Patients were randomly assigned to receive either ivonescimab or tislelizumab, both combined with paclitaxel and carboplatin chemotherapy for four cycles, followed by maintenance therapy with either ivonescimab or tislelizumab for up to 24 months [3].
Key Findings: PD-L1’s Role
While ivonescimab showed an overall benefit, a deeper analysis revealed a crucial connection to PD-L1 tumor proportion score (TPS). Patients with a PD-L1 TPS of under 50% experienced a substantial benefit from the dual blockade approach (hazard ratio [HR] 0.63; 95% confidence interval [CI] 0.41–0.98). However, this improvement was not statistically significant in patients with a PD-L1 TPS of 50% or higher (HR 0.71; 95% CI 0.37–1.33) [2], [4].

PD-L1 (programmed death-ligand 1) is a protein found on some cancer cells. It helps cancer cells evade the immune system. Testing for PD-L1 levels helps doctors predict how likely a patient is to respond to immunotherapy drugs, which work by boosting the immune system to fight cancer.

These findings suggest that PD-L1 status should be a critical factor when deciding on first-line treatment for advanced squamous NSCLC.
