What is the JC Virus?

The JC virus, named after the patient from whom it was first isolated in 1971, is a common human virus spread through the fecal-oral route, appearing in urine and stool of infected individuals 1. Exposure is thought to occur early in life, with blood testing surveys indicating that 50–90 percent of adults have been exposed 1. Initial infection typically occurs without symptoms, establishing a persistent, lifelong infection.

The Threat of Progressive Multifocal Leukoencephalopathy (PML)

For the vast majority, the JC virus remains silent. But, in a small percentage of individuals, the virus reactivates, alters its genetic material, and causes progressive multifocal leukoencephalopathy (PML). PML is a rare but serious neurological disease where the virus actively invades the brain, destroying cells crucial for nerve function, including those that form the protective myelin sheaths 2.

Symptoms and Diagnosis of PML

PML symptoms can mimic other neurological conditions, such as stroke or multiple sclerosis, and may include speech impairments, visual defects, motor dysfunction, and seizures 2. Diagnosis involves identifying signature lesions on brain imaging and detecting JC virus DNA in cerebrospinal fluid 2.

Historical Context and Treatment Advances

PML was first identified in 1958, but became more prevalent in the 1980s among patients with HIV/AIDS. Who is at Risk?

Even as most people carry the JC virus without ever developing PML, individuals with weakened immune systems are at higher risk. This includes those with HIV/AIDS, organ transplant recipients, and people undergoing certain immunosuppressive therapies 2.

Ongoing Research

Researchers continue to investigate the factors that trigger JC virus reactivation and explore potential therapies to prevent and treat PML. Genome-wide association studies are being conducted to identify human genetic determinants of the immune response against polyomaviruses, including BK polyomavirus, JC polyomavirus, and Merkel cell polyomavirus 3. These studies aim to understand the genetic control of these viruses and identify genes and pathways that modulate human immunity.
