Trump Administration Expands Restrictions on Global Health Funding

The Trump administration has significantly expanded the Mexico City Policy, now known as “Protecting Life in Foreign Assistance” (PLFA), to include restrictions on funding for organizations that promote diversity, equity and inclusion (DEI) and what it terms “gender ideology,” in addition to those involved in abortion services. This expansion, announced in January 2026, represents a substantial broadening of the policy’s scope and potential impact on global health programs.

History of the Mexico City Policy

Originally implemented by the Reagan administration in 1984, the Mexico City Policy requires foreign non-governmental organizations (NGOs) to certify that they will not “perform or actively promote abortion as a method of family planning” as a condition of receiving U.S. Global family planning assistance. The policy has been repeatedly rescinded by Democratic administrations and reinstated by Republican administrations CNN.

Initially, the policy applied to approximately $300-$600 million in annual U.S. Funding for family planning. In 2017, President Trump expanded the policy to encompass the majority of U.S. Bilateral global health assistance, increasing the potential funding subject to restrictions to over $7 billion NPR.

The Latest Expansion: DEI and “Gender Ideology”

Vice President JD Vance announced the latest expansion of the policy at the annual March for Life rally on January 23, 2026 NBC News. The new rules, released on January 27, 2026, prohibit funding to organizations that promote “discriminatory equity ideology,” including activities related to DEI, and promote “gender ideology,” encompassing services like gender-affirming care and legal protections based on gender identity NPR.

The expansion applies to most non-military foreign assistance and a wide range of recipient organizations, extending beyond foreign NGOs to include international organizations and U.S. NGOs CNN.

Financial Impact

The Kaiser Family Foundation (KFF) estimates that as much as $39.8 billion in U.S. Foreign aid and nearly 2,600 recipient organizations could be affected by the expanded policy NPR. This figure represents a floor, as the policy’s requirements are often passed down to sub-recipients.

Policy Names and Evolution

The policy has undergone several name changes throughout its history. Originally known as the Mexico City Policy, it was renamed “Protecting Life in Global Health Assistance” (PLGHA) in 2017, and now “Protecting Life in Foreign Assistance” (PLFA) as part of the broader “Promoting Human Flourishing in Foreign Assistance (PHFFA)” Policy umbrella NPR.
