New Blood Test Shows Promise for Early Pancreatic Cancer Detection

Researchers supported by the National Institutes of Health (NIH) have developed a new blood test to identify pancreatic ductal adenocarcinoma, a particularly lethal form of cancer. The test aims to improve early diagnosis and patient outcomes, as pancreatic cancer is often detected at advanced stages when treatment options are limited.

The Challenge of Pancreatic Cancer Diagnosis

Pancreatic cancer has a poor prognosis, with approximately 10% of patients surviving more than five years after diagnosis. Early detection is crucial for improving treatment effectiveness, but currently, there are no widely available reliable screening tools.

How the New Blood Test Works

A team from the University of Pennsylvania Perelman School of Medicine and the Mayo Clinic analyzed blood samples from patients with and without pancreatic cancer to identify potential biomarkers. Initial investigations focused on carbohydrate antigen 19-9 (CA19-9) and thrombospondin 2 (THBS2), but these markers have limitations. CA19-9 can be elevated in benign conditions like pancreatitis, and genetic factors can prevent some individuals from producing it.

Researchers discovered that two novel protein indicators, aminopeptidase N (ANPEP) and polymeric immunoglobin receptor (PIGR), were elevated in patients with early-stage pancreatic cancer. Combining these with CA19-9 and THBS2 created a four-marker panel with an overall identification accuracy of 91.9% and a 5% false positive rate for non-cancer samples. The detection rate among patients with stage I and II pancreatic cancer was 87.5%.

Improved Accuracy and Differentiation

Study leader Kenneth Zaret noted that incorporating ANPEP and PIGR significantly enhances the ability to detect pancreatic cancer at a treatable stage. The test can as well differentiate pancreatic cancer from other pancreatic diseases, such as pancreatitis, reducing the risk of misdiagnosis.

Next Steps and Future Research

Further verification in larger populations is needed, particularly in high-risk groups before symptoms develop, to evaluate the test’s feasibility as a screening tool. Pancreatic ductal adenocarcinoma is the most common form of pancreatic cancer, accounting for over 80% of cases.
