According to recent reports from Nursing in Practice, roughly one in five women of reproductive age used an IUD as of 2023, representing a threefold increase from 2007 to 2010. As adoption rises among younger patients who have not given birth, clinical data shows that pain profiles during insertion remain a significant hurdle for patients.

Pain Rates and Historical Context

Recent studies indicate that about half of patients undergoing IUD placement report intense pain, while another 47 percent experience light or moderate pain. Only 2.5 percent of patients report no pain at all, with the highest pain scores recorded among individuals who have not given birth. Despite decades of patient reports regarding severe discomfort—with some comparing the pain unfavorably to childbirth—systemic changes in pain management have lagged.

Historically, gynecological instruments have changed very little over the past century. Standard tools like the tenaculum—a surgical instrument featuring two hooks at the ends—have roots stretching back to Civil War-era medical implements. According to Alison Edelman, MD, MPH, an ob-gyn specializing in complex family planning and a professor of obstetrics and gynecology at Oregon Health & Science University, traditional instruments often resemble torture devices, highlighting a historical lack of innovation in patient comfort.

New Medical Devices and Regulatory Updates

Recent regulatory approvals and clinical guidance aim to address these longstanding concerns. In 2023, the Swiss start-up Aspivix received FDA approval for the Carevix, a specialized IUD insertion tool. Early patient trials conducted in Europe demonstrated up to 73 percent less pain and up to 83 percent less bleeding compared to traditional methods.

In August of this year, the Centers for Disease Control and Prevention (CDC) updated its clinical guidelines for IUD insertion. The updated guidelines emphasize the necessity for healthcare providers to proactively discuss and offer pain management options. According to Dr. Edelman, the updated framework gives clinicians a practical roadmap for patient-centered communication and shared decision-making.

Katharine White, MD, chief of obstetrics and gynecology at Boston Medical Center, notes that a broader cultural shift has taken root over the past five to ten years. This shift prioritizes patient experiences, needs, and preferences, ensuring that clinical providers listen to concerns rather than pushing specific birth control methods without adequate pain relief.

The updated CDC guidance establishes a clearer standard of care while empowering patients to request specific accommodations, such as stronger pain medications or scheduled pauses during the procedure. Medical experts emphasize that these resources help bridge the gap between patient expectations and clinical execution. While the field of gynecology has historically evolved slowly, the combination of novel devices like the Carevix and updated federal guidelines signals a tangible shift toward prioritizing patient comfort during routine reproductive health procedures.
