Oska Health Secures €11 Million to Expand AI-Powered Chronic Care Platform

Oska Health, a Frankfurt-based healthtech startup, has closed a €11 million seed funding round to scale its AI-powered hybrid care model for patients managing multiple chronic conditions. The funding round was led by Capricorn Partners via their Capricorn Healthtech Fund II and SwissHealth Ventures, with participation from Revent, Calm Storm, LBBW Venture Capital, BMH, GoHub Ventures, and Aurum Impact.

Addressing the Gap in Chronic Disease Management

Oska Health focuses on individuals with multimorbidity – those living with multiple chronic conditions such as chronic kidney disease (CKD), diabetes, and hypertension. The company’s approach combines certified health coaches with an AI-enabled platform to provide continuous support between traditional doctor visits. This model aims to improve adherence to treatment plans, enhance health literacy, and ultimately reduce complications and avoidable hospitalizations.

How Oska Health Works

The platform delivers care through video calls, chat, and a dedicated digital therapy app. Health coaches assist patients translate physician recommendations regarding lifestyle and dietary changes into practical, everyday habits. The AI component supports coaches by reducing administrative burdens and enabling scalable, high-quality care. According to Oska Health CEO and co-founder Niklas Best, “Chronic diseases cannot be managed in isolated doctor visits – they are decided in everyday life. We combine human support with AI to close this gap.”

Expanding Access Through Health Insurance Partnerships

Oska Health already partners with over 20 statutory health insurance funds in Germany, offering its services free of charge to insured members. The company is approved as a certified medical product and has been improving patient outcomes for over three years.

The Need for Continuous Care

Research indicates that approximately 50% of chronically ill patients do not take medications as prescribed, and around 80% struggle to maintain long-term lifestyle changes. Markus Rommel, Principal at SwissHealth Ventures, emphasized the importance of addressing care gaps “where it has had the least impact so far – between doctor’s visits.” Antoine D’Hollander, Investment Director at Capricorn Partners, highlighted the team’s success in securing partnerships with health insurance companies.

With the modern funding, Oska Health plans to expand its team, further develop its technological infrastructure, and scale its reach through partnerships with health insurance companies and medical networks.
