The observational research evaluated electronic health records from the United Kingdom to uncover potential secondary benefits of metabolic treatments like Ozempic and Wegovy.

Led by researchers at Imperial College London, the investigation examined electronic medical records across four separate parallel cohorts, each containing roughly 20,000 to 22,000 patients with underlying airway diseases. According to Chloe Bloom, a respiratory epidemiologist and clinical associate professor at the National Heart & Lung Institute, the team sought to determine whether starting a GLP-1 medication changed the frequency of acute attacks in patients managing both metabolic dysfunction and chronic lung conditions.

Among the individual therapies evaluated within the drug class, semaglutide demonstrated the most pronounced clinical association. Bloom noted that semaglutide use corresponded with a nearly 40% reduction in asthma attacks, alongside a 20% reduction in COPD flare-ups, when compared with similar patients prescribed sulfonylureas, an older class of oral diabetes medications.

Real-World Data Highlights Metabolic Link

The findings point to an underlying connection between metabolic health and respiratory function. Alexander Mathioudakis, a senior lecturer in respiratory medicine at the University of Manchester and chair of the European Respiratory Society’s Group on Airway Pharmacology and Treatment, who was not involved in the research, emphasized that obesity and metabolic dysfunction frequently complicate airway diseases but remain under-recognized in routine care.

Because GLP-1 receptor agonists are known to produce systemic anti-inflammatory effects, researchers hypothesized that these mechanisms might extend benefits to lung tissue. However, because the study relied on retrospective electronic health records rather than randomized controlled trials, investigators established an association rather than direct causation.

Clinical Guidance and Future Research

Study authors strongly cautioned that patients should not initiate GLP-1 therapies solely to treat asthma or chronic obstructive pulmonary disease outside of established prescribing guidelines for type 2 diabetes and chronic weight management. Confirming these respiratory benefits will require dedicated clinical trials that explicitly incorporate lung function metrics, symptom scores, and exacerbation rates as primary endpoints.
