H5N1 avian influenza remains a meaningful zoonotic threat with pandemic potential, despite the current rarity of human cases. The virus’s global spread in birds and mammals, coupled with its ability to mutate, keeps the scientific community and health authorities vigilant.
Development of specific H5 vaccines is progressing, though unevenly across countries and facing regulatory, political, and logistical hurdles.
According to a Nature publication from the summer of 2023,Finland exemplified proactive preparation against avian flu. an H5N1 outbreak impacted fur farms housing mink, foxes, and raccoon dogs, leading to the culling of nearly half a million animals. While no human infections occurred, the risk of viral genetic reorganization in these high-density animal environments prompted health authorities to take an unprecedented European step: offering H5N1 vaccination to high-risk groups, including farm workers, veterinarians, and laboratory personnel.
“We were concerned that, in circumstances where many animals are confined in small spaces, the pathogen could reorganize and become a new pandemic virus,” explained Hanna Nohynek, chief physician at the Finnish Institute of Health and Welfare.Finland became the first, and so far only, country to implement a preventive human vaccination strategy against avian flu.
Vaccine development
The vaccine used, developed by CSL Seqirus, targets clade 2.3.4.4b of the H5 virus, the globally dominant lineage since 2018. This clade has demonstrated an unprecedented ability to infect diverse bird and mammal species, raising particular concern. However,it isn’t currently easily transmitted between people. In the United States,for exmaple,only 70 human cases of H5N1 had been reported as of July 2025,all linked to occupational exposure,primarily in the livestock sector.
Despite this low incidence, experts caution that the risk should not be underestimated. Historically, human avian influenza infections have had very high fatality rates. Since 2003, approximately 1,000 people worldwide have been infected with H5N1, with about half succumbing to the virus. Although the current clade appears to cause milder illness, even reduced mortality could be devastating if the virus gains sustained human-to-human transmission.
