Second Pregnancy Uniquely Alters Mothers’ Brains, Enhancing Attention and Maternal Mental Health

A second pregnancy causes distinct changes in a woman’s brain, building upon the adaptations that occur during the first, according to research from Amsterdam UMC. These changes, particularly in brain networks related to attention and responding to stimuli, may help mothers manage the demands of caring for multiple children and could offer insights into postpartum mental health.

How Pregnancy Changes the Brain

Previous research established that a first pregnancy significantly alters brain structure and function 1. These changes primarily occur in the Default Mode Network, a brain region crucial for self-reflection and social processes. Now, a new study published in Nature Communications reveals that a second pregnancy doesn’t simply repeat these changes, but builds upon them in a unique way 2.

Key Findings from the Amsterdam UMC Study

Researchers monitored 110 women – first-time mothers, women experiencing their second pregnancy and a control group who remained childless. Brain scans before and after pregnancy revealed:

  • Similar but Less Dramatic Changes in the Default Mode Network: The Default Mode Network changes again during a second pregnancy, but to a lesser extent than during the first, suggesting an initial adaptation during first-time motherhood that is then refined.
  • Increased Changes in Attention and Stimuli Response Networks: A second pregnancy leads to more pronounced changes in brain networks responsible for attention control and responding to external stimuli. Researchers suggest this could be beneficial for mothers caring for multiple children 3.
  • Unique Neural Transformations: The study demonstrates that a second pregnancy uniquely changes a woman’s brain, entailing both convergent and distinct neural transformations 2.

The Link to Maternal Mental Health

The study as well uncovered connections between these brain changes and maternal mental health. Structural changes in the brain’s gray matter were associated with peripartum depression – depression occurring during pregnancy or after childbirth. The relationship differed between first-time and second-time mothers:

  • First-Time Mothers: Brain changes were more strongly linked to mental health after giving birth.
  • Second-Time Mothers: Brain changes were more closely related to mental health during pregnancy.

Globally, approximately 10% of pregnant women and 13% of those who have recently given birth experience mental health challenges, primarily depression 3.

Implications for Maternal Healthcare

“With this, we have shown for the first time that the brain not only changes during the first pregnancy, but also during the second,” says Elseline Hoekzema, head of the Pregnancy Brain Lab at Amsterdam UMC 1. “Each pregnancy leaves a unique mark on the female brain.” Researchers believe these findings could improve maternal healthcare by enhancing understanding of the development of postpartum depression and how the brain adapts to motherhood 4.
