Nine-Year-Classic Boy First in UK to Undergo Pioneering Leg-Lengthening Surgery
Alfie Phillips, a nine-year-old boy from Northampton, has become the first person in the United Kingdom to undergo a groundbreaking surgery to lengthen his leg. The procedure offers new hope for children born with significant limb length discrepancies, providing an alternative to more invasive treatments.
What is Fibular Hemimelia?
Alfie was born with fibular hemimelia, a rare condition affecting fewer than one in 40,000 births [1]. This condition results in the incomplete or absent development of the fibula, the smaller bone in the lower leg. In Alfie’s case, his right leg was more than an inch shorter than his left, and specialists predicted it would grow to be six centimeters shorter by the time he reached sixteen [2].
The Pioneering Surgery at Alder Hey Children’s Hospital
Prior to this new technique, the only treatment option for Alfie was an external frame fixed to his bone with pins or wires, which carries risks of pain, infection, and scarring [1]. Surgeons at Alder Hey Children’s Hospital in Liverpool pioneered a new approach, implanting a lengthening nail on Alfie’s right thigh bone. This nail was slowly pulled apart over time using magnets, stimulating new bone growth and gradually increasing his leg length by 3cm [1].
Alfie’s Experience and Recovery
Alfie described feeling “excited” and scared to be the first to undergo the procedure, but almost a year later, he is “running around as normal” and enjoying playing basketball [1], [2]. He noted that before the surgery, his schoolmates would comment on his different gait, and he found it “annoying” when conversations often revolved around his condition [2]. Now, he says, he can “tower over” his friends [2].
Expanding Access to the Procedure
Since Alfie’s successful surgery, Alder Hey has performed the technique on three other children with fibular hemimelia [1]. Lengthening nails have been used in adults, but this procedure was previously considered too risky for younger children due to potential damage to growth plates. Other centers around the country are now preparing to adopt the technique [1].
A Remarkable Recovery
Nick Peterson, a consultant orthopaedic surgeon at Alder Hey, described Alfie’s recovery as “amazed” and “unheard of,” noting that his pain score dropped to zero within seven days and he required only occasional paracetamol [2].
