Researchers analyzing data from thousands of participants found that higher blood plasma levels of xylitol correlated with a higher incidence of heart attacks, strokes, and death over long-term follow-up periods.

What Is Xylitol and How Does It Affect the Body?

Xylitol is a sugar alcohol that occurs naturally in the human body as a byproduct of normal glucose metabolism, and it is found in very small quantities in some foods. As an artificial sweetener, however, it is added to commercial foods, beverages, and oral hygiene products in amounts often exceeding natural levels by a factor of 1,000 or more. According to Dr. Marco Witkowski of the Charité – Universitätsmedizin Berlin, previous research demonstrated that xylitol can increase the clotting ability of blood platelets. Furthermore, elevated blood levels of xylitol had previously been linked to cardiovascular events in high-risk patients over a three-year window.

Long-Term Findings From the Canadian and European Cohorts

To investigate whether this risk extends to the general population over extended periods, researchers examined health data from 17,710 participants across two major observational studies: the Canadian Longitudinal Study on Aging (CLSA) and the European Prospective Investigation into Cancer and Nutrition (EPIC-Norfolk) study. The research team compared individuals with the highest blood xylitol levels against those with the lowest levels, tracking serious cardiovascular events defined as heart attack, stroke, or death.

The results showed a clear, step-wise escalation in risk. In the Canadian cohort, which followed participants for six years, the risk of a major cardiac event was 57 percent higher for individuals with the highest xylitol levels compared to those with the lowest levels. This calculation adjusted for established cardiovascular risk factors such as age, sex, smoking status, blood lipids, diabetes, and hypertension. In the EPIC-Norfolk cohort, which tracked participants over a 30-year span, the observed risk difference was 18 percent. The correlation remained consistent regardless of body mass index, blood pressure, or blood lipid profiles—the very clinical traits that frequently prompt individuals to select low-calorie sugar substitutes.

Context and Regulatory Safety Standards

Despite the findings, researchers emphasize that the observational study establishes a strong statistical association rather than a direct causal link. Artificial sweeteners remain widely consumed by individuals seeking alternatives to sugar and have historically passed regulatory safety evaluations. Professor Dan Atar, a member of the communications committee for the European Society of Cardiology, noted that artificial sweeteners are ubiquitous in modern society and that their safety assessments have been thoroughly reviewed and confirmed by regulatory agencies. However, as Dr. Witkowski points out, these long-term population results underscore gaps in current knowledge regarding xylitol’s cardiovascular safety profile, pointing to an urgent need for further investigation as consumption rates continue to climb.
